DİSTAL HİPOSPADİAS ONARIMINDA TIPU TEKNİĞİ: 8 YILLIK DENEYİMİMİZ

Öz



Amaç: Hipospadias penil üretranın anormal
gelişimi sonucu eksternal meanın normal lokalizasyonunda olmayıp hemen glansın
altında, penis gövdesi, skrotum veya perineumda bulunması ile karakterize
konjenital bir anomalidir. Bu çalışmada son 8 yıl içinde kliniğimize distal
hipospadias nedeni ile başvuran ve TIPU yöntemi ile üretroplasti operasyonu
yapılan hastaların değerlendirilmesi amaçlandı.

Gereç ve Yöntemler:
2010-2018 yılları
arasında TIPU yöntemi ile üretroplasti yapılan 129 hipospadiaslı olgunun
kayıtları geriye dönük olarak incelendi. Hastaların yaşları, hastanede kalış
süreleri, ortalama takip süreleri, eksternal meanın yerleşimi (glandüler,
koronal, subkoronal, distal penil ve midpenil), kordi varlığı, stent süreleri,
ameliyat sonrası fistül gelişimi, meatal stenoz ve striktür gibi olası
komplikasyon çeşitleri değerlendirildi.

Bulgular: Hastalarımızın 102’si primer olgu iken
(%79), 27’si sekonder olguydu (%21). Operasyon sonrası başarı oranı primer ve
sekonder olgularda sırası ile %85.3 ve %77.7 olarak bulundu. Komplikasyon oranı
ise sırası ile %14.7 ve %22.2 olarak tespit edildi.







Sonuç: TIPU tekniği
primer ve sekonder distal hipospadias olgularında başarı ile uygulanacak bir
yöntemdir.

TIPU Technique in Distal Hypospadias Repair: Experience of 8 Years

Öz

Objective: Hypospadias is a congenital disorder
due to abnormal development of the penile urethra resulting in abnormal
localization of the external urethral meatus in the penile corpus, scrotum or
perineum. In this study, we aimed to evaluate the patients who underwent
urethroplasty with TIPU technique due to hypospadias in the last 8 years
period.

Material and Methods: Medical records of 129 patients with a
history of urethroplasty with TIPU technique between the years 2010-2018 were
evaluated retrospectively. Age of the patients, duration of hospital stay and
postoperative follow-up, localization of the external meatus (glanular, coronal,
subcoronal, distal penile and midpenile), presence of chordee, duration of
urethral stenting and the rate of complications like fistula formation, meatal
stenosis and stricture were recorded.

Results: A hundred and two of the cases were
primary (79%) while 27 of the cases were recorded as secondary (21%).
Postoperative success rate was recorded as 85.3%in primary and 77.7%in
secondary cases. Complication rates were found as 14.7% and 22.2% respectively.







Conclusion: TIPU technique is a safe and effective
method which can be applied successfully in the treatment of primary and
secondary hypospadias cases.
