Hemodiyaliz Hastalarında Sıvı Kontrolü İle Semptom Şiddeti İlişkisi

Abstract

Amaç: Bu çalışma, hemodiyaliz hastalarında sıvı kontrolü ve semptom şiddetini belirlemek, ilişkili faktörleri ortaya koymak, sıvı kontrolü ve semptom şiddeti arasındaki ilişkiyi saptamak amacıyla yapılmıştır.
Yöntemler: Tanımlayıcı ve ilişki arayıcı olarak gerçekleştirilen araştırma, 01-30 Nisan 2022 tarihleri arasında İstanbul’da özel bir hemodiyaliz merkezinde tedavi gören 150 hasta ile gerçekleştirilmiştir. Veriler hasta tanılama formu, Hemodiyaliz Hastalarında Sıvı Kontrol Ölçeği (HHSKÖ) ve Diyaliz Semptom İndeksi (DSİ) kullanılarak elde edilmiştir. Verilerin değerlendirilmesinde Mann Whitney U testi, Kruskal Wallis testi ve Spearman Korelasyon analizi kullanılmıştır.
Bulgular: Hastaların HHSKÖ puan ortalaması 50,75±8,13’tür. Hastaların yaşı arttıkça sıvı kontrolüne yönelik bilgi düzeyinin azaldığı, bununla birlikte kadın, diyetine dikkat eden ve sağlık kontrolünü düzenli yaptıran hastaların sıvı kontrol düzeyinin daha yüksek olduğu tespit edilmiştir (p<0,05). Hastaların DSİ puan ortalaması 19,09±17,08’dir. Sağlık kontrollerine ailenin zorlaması ile giden, rutin kontrollerini yaptırmayan ve diyetine uymayan hastaların semptom şiddetinin yüksek olduğu belirlenmiştir (p<0,05). Ayrıca HHSKÖ ile DSİ puan ortalamaları arasında negatif yönlü zayıf düzeyde ilişki olduğu bulunmuştur (r = -0,349; p<0,05).
Sonuç: Hemodiyaliz hastalarının sıvı kontrolünün genel olarak orta düzeyde olduğu, semptom şiddetinin ise oldukça düşük olduğu, sıvı kontrolüne yönelik bilgi, davranış ve tutum düzeyi arttıkça semptom düzeyinin azalabileceği belirlenmiştir. Hemşirelerin hemodiyaliz hastalarına sıvı kontrolünün önemi konusunda düzenli aralıklarla eğitim vermesi ve danışmanlık yapması, sıvı kontrolüne uyumda hastanın yakından takip edilmesi önerilmektedir.

Relationship Between Fluid Control and Symptom Severity in Hemodialysis Patients

Abstract

Objective: This study was conducted to determine fluid control and symptom severity in hemodialysis patients, to reveal related factors, and to determine the relationship between fluid control and symptom severity.
Methods: The descriptive and correlational study was conducted with 150 patients receiving treatment in a private hemodialysis center in Istanbul between 01-30 April 2022. Data were obtained using a patient identification form, the Fluid Control Scale in Hemodialysis Patients and the Dialysis Symptom Index.
Results: The mean Fluid Control Scale in Hemodialysis Patients score of the patients was 50.75±8.13. The mean Dialysis Symptom Index score of the patients was 19.09±17.08. In addition, it was found that there was a weak negative correlation between the mean scores of the Fluid Control Scale in Hemodialysis Patients and the Dialysis Symptom Index (r=-0.349; p<0.05).
Conclusion: It has been determined that the fluid control of hemodialysis patients is generally at a moderate level, the symptom severity is quite low, and the symptom level may decrease as the level of knowledge, behavior, and attitude toward fluid control increases. It is recommended that nurses should regularly educate and counsel hemodialysis patients about the importance of fluid control and closely monitor the patient's compliance with fluid control.
