Hidroksiklorokinin tetiklediği akut generalize ekzantematöz püstülozis: Literatür eşliğinde bir olgu

Akut
generalize ekzantematöz püstülozis (AGEP), genel olarak ilaç ilişkili bir
reaksiyon olup, ödemli eritem zemininde akut gelişen, folliküler olmayan, steril
püstüller ile karakterize bir tablodur. Hidroksiklorokin
(HCQ), antimalaryal bir ilaç olup romatizmal ve dermatolojik hastalıkların
tedavisinde yaygın olarak kullanılmaktadır ve AGEP’in nadir nedenlerinden
biridir. 44 yaşında kadın hasta romatoid artrit nedenli hidroksiklorokin
alırken gelişen AGEP, HCQ’nin kesilmesi ve steroid tedavisinin başlanması ile düzeldi.
Bu olgudaki amacımız romatoloji pratiğinde sık kullanılan bir ilaç olan HCQ kullanımı
sonrası gelişebilecek oldukça nadir istenmeyen bir durum olan AGEP ile ilişkili
farkındalığı arttırmak ve bu istenmeyen etkinin yönetimini belirlemek.

Acute generalized exanthematous pustulosis (AGEP)
is a clinical reaction pattern that is generally drug induced and characterized
by acute, nonfollicular sterile pustules on a background of edematous erythema.
Hydroxychloroquine (HCQ) is an
antimalarial drug which is widely used in dermatologic and rheumatologic
diseases and reported as a rare cause of AGEP. A
44 years old female patient developed AGEP while recieving HCQ for rheumatoid
arthritis and resolved after cessation of HCQ and starting steroid therapy. The
aim of this case is to raise the awareness about AGEP which is an uncommon and
undesired effect of HCQ; a widely used drug in rheumatology practice.
