An 85-year-old male with severe right lower quadrant abdominal pain, nausea, and vomiting for twenty-four-hour presented to the emergency department. He had undergone an ERCP two months previously for primary choledocholithiasis. Sphincterotomy was performed, and a 10 Fr 10 cm plastic biliary stent was placed in the common bile duct. On physical examination at admission, he was tenderness in the right lower quadrant. Abdominal computed tomography showed an extra-luminated biliary stent accompanied by localized free air and fluid in the right lower quadrant. The patient underwent emergency surgery. Most of the migrated stents are excreted by passage, but it can cause mortal complications such as bowel perforation, intra-abdominal abscess, and ileus.
