AB  - Gunshot injuries pose clinical and medicolegal challenges, with morbidity and mortality closely linked to the location and severity of the trauma. Prompt assessment and timely intervention are crucial.We present a case of a shotgun-related injury involving a plastic cartridge wad, undetectable by imaging but incidentally discovered embedded in the thoracic wall during surgery. If left untreated, it could have resulted in complications such as empyema. The patient, with a Glasgow Coma Scale score of 11, exhibited a lateral thoracic wall hematoma, comminuted rib fractures, and radiologically visible shotgun pellets. Posterolateral thoracotomy revealed the wad within the chest wall musculature, which was surgically removed. The patient was discharged without complications on postoperative day 12.This case illustrates that not all foreign bodies in firearm injuries are metallic. Radiolucent materials may be missed and pose infectious risks. It also provides valuable information for estimating shooting distance and contributes to forensic assessment.

N2  - Ateşli silah yaralanmaları, yaralanmanın yeri ve ciddetiyle yakından ilişkili morbidite ve mortalite nedeniyle önemli klinik ve adli zorluklar ortaya koymaktadır. Bu nedenle, hızlı değerlendirme ve uygun müdahale hayati önem taşır.     Bu yazıda, görüntüleme yöntemlerinde fark edilemeyen ancak cerrahi sırasında toraks duvarına saplanmış şekilde tesadüfen saptanan tüfek fişeğine ait plastik şarapnel parçasına bağlı bir yaralanma olgusu sunulmuştur. Bu yabancı cisim çıkarılmadığı takdirde ampiyem gibi komplikasyonlara neden olabilir. Glaskow Koma Skalası 11 olan hastada, toraks lateral duvarında hematom, parçalı kaburga kırıkları ve saçma taneleri mevcuttu. Posterolateral torakotomi sırasında, plastik fişek tapası göğüs kasları içerisinde saptanarak çıkarılmıştır. Postoperatif dönemde komplikasyon gelişmeyen hasta 12. günde taburcu edilmiştir.     Bu olgu, ateşli silah yaralanmalarında her zaman yalnızca metal içerikli yabancı cisimlerin olmayabileceğini ve görüntülemede fark edilemeyen bu cisimlerin enfeksiyon ile ampiyem gibi komplikasyonlara yol açabileceğini hatırlatmaktadır. Ayrıca, yakın mesafe yaralanmalarının tespiti ve adli tıbbi değerlendirme açısından da önemli bilgiler sunmaktadır.
