AB  - Bu derleme, duodenum ülserinin fizyopatolojisini ele alarak mide asiditesi, vagus sinir uyarımı ve gastrin üretiminin etkilerini vurgular. Cerrahi uygulamaların tarihsel gelişimi Billroth I/II yöntemlerinden total/selektif/yüksek selektif vagotomi ve kombine ameliyatlara kadar incelenir. Modern yöntemlerin (örneğin selektif mide vagotomisi + antrektomi + Billroth I rekonstrüksiyonu) fizyolojik açıdan üstün ve nüks oranı düşük alternatifler sunduğu gösterilmiştir. Ameliyat sonrası döküntü sendromu, vagotomi sonrası diyare ve diğer komplikasyonlar detaylandırılır. Çalışma, antrum motilitesini koruyarak asit yükünü azaltmanın cerrahide ideal yaklaşım olduğunu savunur.
