N2  - The Arnold-Chiari-I malformation (ACM) is a congenital anomaly characterized by herniation of the cerebellar tonsils into the spinal canal. The anomaly may present in a variety of ways and at times with vague symptoms. A 40-year-old female patient referred to our clinic with complaints of acute onset walking diffuculty, blurred vision, headache and diplopia. Neurological examination revealed right internuclear ophthalmoplegia. We present the unusual case of a patient suffered from blurred vision, ataxia and right INO as a finding of ACM which improved with only, at the right eye introspection remained restricted to left, after decompression surgery.
