Extradural Elektif Sezaryenlerde Lidokain+Bupivakain Mixt Alkalinize Lokal Anestezik Solüsyonunun Bolus ve Aralıklı Dozlarda Verilmesinin Toksisiteye Etkisinin Karşılaştırılması

Abstract

Çalışmalarımızda epidural aralığa ilacın veriliş yönteminin toksisiteye bir etkisinin olup olmadığını incelemeyi amaçladık. Elektif seksiyolarda, fetal distresi olmayan, miadında gebeleri çalışma grubu (n:10) ve kontrol grubu (n:10) olarak iki gruba ayırdık. Çalışma grubuna ilacı test dozundan sonra 5 ml aralıklı dozlarda, kontrol grubuna ise tek doz bolus olarak verdik. Gruplar arasında ilacın veriliş yöntemi hariç diğer yöntemlerde fark yoktu. Anestezist ve hasta gözüyle gruplar arasında blok kalitesi yönünden istatiksel olarak anlamlı fark yoktu. Gruplar arasında pik etki zamanı ortalamaları açısından istatiksel olarak anlamlı fark vardı (p<0.05). Her iki grupta da intraoperatif sistemik toksik etkiyi destekleyecek bir bulguya rastlanmadı. Elektif seksiyolarda mixed alkalinize solüsyonun konsantrasyonun azalması sebebi ile toksik etkiye faydalı olduğu ve aralıklı dozlarda ilaç yapmanın daha emin bir yol olduğu kanısındayız.

A Comparison Between the Toxic Effect of Mixed of Lignocaıne+Bupivacaine Alkalinized Local Anesthetic Solutions at Bolus and at Interval Doses in Extradural Elective Caeserean Sections

Abstract

We attempted in this study to investigate whether the way the drugs were administered had any effect on toxicity. We divided in term who had no fetal distress into two groups: working group and control group in elective caesarean sections. As the method of administering the drug into extradural space in those patients in the working group, at fractioned doses of 5 ml following the test dosage. However, in the control group was administered at a single bolus dosage. There was no other difference between groups except the method of administering the drugs. There was also no statistically significant difference as regard the block quality between the anaesthetists and patients opinions. There was a statistically significant difference as regards the average peak effect time between groups (p<0.05). We believe that alkalinized of mixture solution is benefical due to decrease of concentration and that it is a safer method in the administration of fractioned doses.
