Airtraq, Nazotrakeal Airtraq, Airtraq ile fiberoptik bronkoskop kombinasyonunun orotrakeal entübasyonda karşılaştırılması: prospektif, randomize klinik çalışma

Abstract

Amaç: Zor havayolun için standart Airtraq efektif bir video laringoskoptur. Ancak Airtraq, çok az sayıda zor havayolu hastasında başarısız olmaktadır. Fiberoptik entübasyon, beklenen zor havayolunda altın standarttır. Airtraq ile entübasyon, fiberoptik bronkoskop yardımı ile iyileştirilebilir. Nazotrakeal Airtraq, nazotrakeal entübasyon için üretilmiştir. Oysa, bazı vaka sunumları Nazotrakeal Airtraq’in orotrakeal entübasyonda da kullanılabildiğini göstermiştir. Bu çalışmanın amacı, standart Airtraq, Nazotrakeal Airtraq ve stileli tüp kombinasyonu, standart Airtraq ile fiberoptik kombinasyonunun entübasyon sürelerini karşılaştırmaktır.
Materyal ve metod: Araştırma Etik Komite onamı ve tüm hastalardan yazılı aydınlatılmış hasta onamı alındıktan sonra, orotrakeal entübasyon gerektiren elektif cerrahiye girecek ASA I-II 120 hasta bu prospektif randomize çalışmaya dahil edildi. Hastalar; standart Airtraq (Group A), Nazotrakeal Airtraq ve stileli tüp kombinasyonu (Group N), standart Airtraq ile fiberoptik kombinasyonu (Group A+F) olarak üç gruba ayrıldı. Optimizasyon ve tüp ilerletme manevraları; cihazın yeniden yerleştirmesi, cihazın az geri çekilmesi, krikoid bası, tüpün 90° saat tersine rotasyonu manevrası kaydedildi. Cormack-Lehane evreleri, yerleştirme süreleri, entübasyon ve toplam entübasyon süreleri de kaydedildi. Hemodinamik değişiklikler ve postoperatif minor komplikasyonlar da kaydedildi.
Sonuçlar: Hastaların demografik verileri ve havayolu karakteristikleri benzerdi. Grup N, grup A dan daha hızlı yerleştirildi (p<0.001). Toplam entübasyon ve entübasyon süreleri Grup N e kıyasla Grup A+F de daha uzundu (p<0.001). Manevra gereksinimi, hemodinamik parametreler ve postoperative minor komplikasyonlar açısından gruplar benzerdi.
Tartışma: Normal havayoluna sahip erişkin hastalarda Fiberoptik bronkoskop ile combine edilmiş standart Airtraq, standart Airtraq’in tek başına kullanımından ve Nazotrakeal Airtraq ile stileli tüp kombinasyonundan daha uzun entübasyon süresi sağlamaktadır.
