Bir Komplike Kuron-Kök Kırığı Olgusunun Cerrahi Ekstrüzyon ve 180˚ Rotasyonla Alveol İçi Transplantasyonu

Abstract

Travmatik dental yaralanmalar, genel populasyonda tüm sağlık yaralanmalarının %5'ini, pediatrik populasyonda ise %17'sini oluşturur. Kron-kök kırıklı dişlerin restorasyon aşamasında birçok farklı sorunla karşılaşılır. Kron-kök kırıklı dişlerde kırığın durumuna bağlı olarak kron boyu uzatma, ortodontik ekstrüzyon, cerrahi ekstrüzyon gibi birçok farklı tedavi seçeneği olmasına karşın, kırık hattının marjinal kemik sınırına ulaştığı birçok vakada dişin çekimine karar verilmesi sık karşılaşılan bir durumdur.
Bu olgu üst çene sağ orta keser dişinde komplike kron-kök kırığı bulunan, 11 yaşındaki hastanın dişine uygulanan cerrahi ekstrüzyon ve 180˚lik rotasyonla birlikte gerçekleştirilen alveol içi (“intra-alveolar”) transplantasyon tedavilerini içermektedir.
İki yıllık takip sonucunda radyografik olarak herhangi bir rezorpsiyon ve periapikal lezyon gelişimi izlenmemiş olup klinik olarak hastanın herhangi bir şikayeti bulunmamaktadır.
Sonuç olarak; olgumuzun iki yıllık başarılı sonuçları göz önünde bulundurularak, kron-kök kırığı bulunan dişlerde ortodontik işlemlerin kontrendike olduğu durumlarda, alternatif bir tedavi yaklaşımı olarak cerrahi ekstrüzyon ve alveol içi transplantasyon tedavilerinin mutlaka değerlendirilmesi önerilmektedir.

Surgical Extrusion and Intra-Alveolar Transplantation with 180˚ Rotation of a Complicated Crown-Root Fracture Case

Abstract

Traumatic dental injuries constitute 5% of all health injuries in the general population and 17% in the pediatric population. Many different problems are encountered during the restoration of crown-root fractured teeth. Although there are many different treatment options such as crown lengthening, orthodontic extrusion, surgical extrusion depending on the condition of the fracture in teeth with crown-root fracture, it is a common situation to decide to extract the tooth in many cases where the fracture line reaches the marginal bone limit.
This case includes surgical extrusion and intra-alveolar (“intra-alveolar”) transplantation treatments performed with 180° rotation of the tooth of an 11-year-old patient with a complicated crown-root fracture in the maxillary right middle incisor.
As a result of the two-year follow-up, no resorption and periapical lesion development were observed radiographically, and the patient did not have any clinical complaints.
As a conclusion; Considering the two-year successful results of our case, surgical extrusion and intra-alveolar transplantation should be evaluated as an alternative treatment approach in cases where orthodontic procedures are contraindicated in teeth with crown-root fracture.
