                                                                                                <abstract><p>Amaç: Hastane dışında gerçekleşen doğumların yer ve nedenlerini tespit etmek, ülkemizde evde doğum seçeneğinin değerlendirilmesi için veri oluşturmak, hastane dışında doğan bebeklerin takibinde dikkat edilmesi gereken noktaları araştırmak.Materyal ve metod: Bu çalışmada, 1 Mart 2014 ve 30 Eylül 2019 tarihleri arasında hastane dışında gerçekleşen 87 doğum geriye dönük olarak incelenmiştir. Doğumun nerede gerçekleştiği ve neden hastane dışında gerçekleştiği kaydedilmiştir. Gebeliğin riskli bir gebelik olup olmadığı Amerikan Jinekoloji ve Obstetrik Derneği kriterlerine göre geriye dönük olarak değerlendirilmiştir. Doğan bebeklerin demografik, klinik ve laboratuvar bulguları analiz edilmiştir. Bebeklerin yatış tanıları, yatış süresi ve mortalitesi, 2019 yılında hastanede doğan 2779 bebek ile karşılaştırılmıştır. İstatistiksel analiz için SPSS Statistics 22.0 (Armonk, New York: IBM Corp.) kullanılmıştır.Bulgular: Genel olarak hastane dışında doğum yapan gebelerin çoğunun (n=52, %59.7) takipsiz olduğu, yarıya yakının (n=42, %48.2) sorunlu veya gelişmekte olan ülkelerden göç etmiş yabancı uyruklu kadınlardan oluştuğu kaydedilmiştir. Hastane dışında doğum yapan kadınların 13’ü (%14.9) kendi isteği ile evde doğum yapmayı tercih etmiştir. Bu 13 kadından 11’inin (%84.6) gebeliğinin takipsiz olduğu kaydedilmiştir. İstemeden hastane dışında doğum yapan 74 gebenin 56’sı (%75.6) doğum için hastaneye zamanında yetişememiştir, 11’inde (%14.8) ihmal ve eğitimsizlik, 4’ünde (%5.4) istenmeyen gebelik hastane dışı doğum nedeni olarak ileri sürülmüştür.Gebelerin %32.1’sinin riskli gebelik, %67.9’unun düşük riskli gebelik olduğu kaydedilmiştir. Bebeklerin %6.9’u preterm, %87.3’ü term veya postterm gebelik haftasında doğmuş, %5.7’sinin ise gebelik haftası bilinmemektedir. Ortalama doğum ağırlığı 3270±453 gram olarak saptanmıştır. Hastane dışında doğan bebeklerde, hastanede doğanlara göre yenidoğan sarılığı, makrozomi, hipoglisemi, sepsis ve polisitemiye daha sık rastlanmıştır (p</p></abstract>

                            <p>Objective: To analyze the locations and reasons for out-of-hospital births admitted to our hospital, evaluate the feasibility of planned home birth in Istanbul, Türkiye, andexamine key aspects of follow-up care for newborns delivered outside hospital settings.Methods: This retrospective study included 87 newborns delivered outside a hospital between March 2014 and September 2019 and subsequently admitted to our facility.Birth location and reasons for out-of-hospital delivery were recorded. Pregnancies were classified as high- or low-risk according to the American College of Obstetriciansand Gynecologists criteria. Demographic, clinical, and laboratory data were analyzed. Admission diagnoses, length of hospital stay, and mortality rates were compared withthose of 2,779 newborns delivered in our hospital in 2019. Statistical analyses were performed using SPSS Statistics 22.0 (Armonk, NY: IBM Corp.).Results: The majority of women who delivered outside the hospital (n=52, 59.7%) had not received prenatal care, and nearly half (n=42, 48.2%) were foreign nationalsfrom conflict-affected or developing countries. Among women who delivered at home, 13 (14.9%) did so voluntarily, of whom 11 (84.6%) had not received prenatal care.Among unplanned out-of-hospital births, 56 (75.6%) occurred due to inability to reach a hospital in time.
