                            <p>Nodular scabies  NS  is a rare entity thought that hypersensitivity reaction to remnant of mites or antigens developing after scabies treatment. Pruritic nodules are mostly localized on genital region and scrotum. 21-years-old man admitted with itchy swellings on scrotum. He was diagnosed scabies 2 months ago and treated with %5 permetrin cream. Patient’s body lesions were improved but both scrotal lesions and pruritus got worsen. Dermatologic examination revealed multiple erythematous, smooth surfaced, mildly hard noduls on scrotum. Histopathologically; perivascular infiltration including eosinophils, plasma cells and PMNL were observed on superficial and deep dermis. The patient was diagnosed NS, pimecrolimus cream was started. Pruritus had regressed and lesions partially improved after a week. NS is a difficult to treat and rare disease in dermatologic practice. It may be confused with benign and malign other conditions clinically and histopathologically. NS must be thought in differential diagnosis of scrotal and penil nodular lesions</p></trans-abstract>
                                                                                                                                    <abstract><p>Nodular skabiyez NS , skabiyez tedavisinden sonra vücutta kalan akar parçalarına ve antijenlerine karşı gelişen hipersensitivitenin sorumlu olduğu daha çok genital bölge ve skrotumda yerleşen, kaşıntılı nodüllerle karakterize nadir bir antitedir. 21 yaşında erkek, skrotumda kaşıntılı kabarıklıklarla başvurdu. 2 ay önce skabiyez tanısıyla permetrin %5 krem tedavisi uygulanmıştı. Vücudundaki şikayetleri gerileyen hastanın skrotumdaki lezyonları gerilememiş ve kaşıntısı giderek artmıştı. Dermatolojik muayenesinde peniste ve skrotumda eritemli yüzeyi düzgün, hafif sert multiple nodüller saptandı. Histopatolojisinde daha çok damarlar etrafında yoğunlaşmış yüzeyel ve derin dermiste yoğun eozinofil,seyrek plazmosit ve polimorfonükleer lökosit içeren lenfohistiositik infiltrat saptandı. Bu bulgular eşliğinde NS tanısı konularak pimekrolimus krem başlandı. 1.hafta kontrolünde, kaşıntısında azalma ve lezyonlarında kısmi gerileme saptandı. NS dermatoloji pratiğinde az rastladığımız ve tedavisi zor bir antitedir. Hem klinik hem de histopatolojik olarak benign ve malign durumlarla karışabilir. Skrotum ve penisteki noduler lezyonların ayırıcı tanısında NS düşünülmelidir</p></abstract>
