Retrospective Evaluation of Laboratory Tests of Patients with Rotavirus and Adenovirus Positivity in Kırşehir Province

Abstract

Purpose: Acute gastroenteritis is a major cause of morbidity and mortality in infants and children worldwide. In this study, it was aimed to determine the frequency of rotavirus and adenovirus positivity and retrospective evaluation of laboratory findings in pediatric patients admitted to our hospital with a preliminary diagnosis of acute gastroenteritis.

Materials and Methods: In this study, stool samples and blood test results of pediatric patients admitted to Kırşehir Training and Research Hospital with a preliminary diagnosis of acute gastroenteritis between September 2018 and September 2019 were evaluated retrospectively. Rotavirus and enteric adenovirus antigens were investigated using immunochromatographic test in stool samples

Results: Viral antigen was identified in 229 (16.45%) of 1392 patients. 14.8% (n = 34) of patients was positive for adenovirus while rotavirus was detected in 85.2% (n = 195). 116 (50.65%) were male and 113 (49.35%) were female. The lymphocyte, neutrophil, leukocyte and CRP values of adenovirus positive patients were higher than the reference range in 64.7%, 26.5%, 2.9% and 43.8% of the patients, respectively. The lymphocyte, neutrophil, leukocyte and CRP values of rotavirus positive patients were higher than the reference range in 29.7%, 28.7%, 4.6% and 58.1% of the patients, respectively.

Conclusion: The distinction of viral or bacterial pathogen causing acute gastroenteritis is critical in determining ap-propriate treatment. We believe that determining specific laboratory tests that can detect viral gastroenteritis agents and investigating the viral antigen in the stool will reduce the unnecessary use of antibiotics.

Kırşehir İlinde Rotavirüs ve Adenovirüs Pozitifliği Olan Hastaların Laboratuvar Testlerinin Retrospektif Değerlendirilmesi

Abstract

Amaç: Akut gastroenterit dünya çapında bebek ve çocuklarda morbidite ve mortalitenin önemli bir nedenidir. Bu çalışmada hastanemize akut gastroenterit ön tanısı ile başvuran çocuk hastalarda rotavirüs ve adenovirüs pozitifliği sıklığının saptanması ve laboratuvar bulgularının retrospektif olarak değerlendirilmesi amaçlandı.

Araçlar ve Yöntem: Bu çalışmada Kırşehir Eğitim ve Araştırma Hastanesine akut gastroenterit ön tanısı ile Eylül 2018-Eylül 2019 tarihleri arasında başvuran çocuk hastaların gaita örnekleri ve kan testi sonuçları retrospektif olarak incelendi. Gaita örneklerinde rotavirüs ve enterik adenovirüs antijenlerinin varlığı immunokromatografik test ile araştırıldı.

Bulgular: Çalışmada toplam 1392 hastanın 229’unda (%16.45) viral antijen tespit edildi. Bu olguların %14.8’i (n=34) adenovirus pozitif, %85.2’si (n=195) rotavirus pozitif olarak saptandı. Olguların 116’sı (%50.65)’i erkek, 113 (%49.35)’i kız idi. Adenovirüs pozitif hastaların lenfosit, nötrofil, lökosit ve CRP değerleri, hastaların sırasıyla %64.7, %26.5, %2.9 ve %43.8’inde referans aralığa göre yüksek bulundu. Rotavirus pozitif hastaların lenfosit, nötro-fil, lökosit ve CRP değerleri, hastaların sırasıyla %29.7, %28.7, %4.6 ve %58.1’inde referans aralığa göre yüksek bu-lundu.

Sonuç: Akut gastroenterite neden olan viral ya da bakteriyel patojen etkenin ayrımı uygun tedavinin belirlenmesi açısından kritiktir. Viral kaynaklı akut gastroenterit etkenlerini saptayabilecek spesifik laboratuvar testlerinin belir-lenmesi ve gaitada viral antijen bakılmasının gereksiz antibiyotik kullanımını azaltacağını düşünmekteyiz.
