Abstract

Perianal fistül(PF) ve Müsünöz adenokarsinoma(MA)'nın birlikte görülmesi çok nadirdir. Malign dönüşüme yol açan altta yatan patofizyoloji hala net değildir. MA gelişmesi için PF'nin en az 10 yıl bulunması gerekir ve bu tanı kriterlerinden biridir . Ancak Michihiro Koizumi ve ark. 3 yıllık takip ile PF ile ilişkili bir MA varlığını ortaya koymuşlardır. Bizim olgumuzda PF hastalığı öyküsü 3 yıldı. PF'li hastalarda tanıdan 10 yıl sonra MA gelişme riski olmasına rağmen, MA'nın daha erken dönemde de gelişebileceği akılda tutulmalıdır. Bu nedenle özellikle tedavi edilmeyen ve fistül traktında kronik inflamatuvar süreci devam eden hastalar MA açısından yakından izlenmelidir.

Abstract

It is very rare to see perianal fistula(PF) and mucinous adenocarcinoma(MA) together. The underlying pathophysiology leading to malignant transformation is still not clear. For MA to develop, PF must be at least 10 years old, and this is one of the diagnostic criteria . However, Michihiro Koizumi et al. revealed the existence of a PF-related MA with a 3-year follow-up. In our case, the history of PF disease was 3 years. Although there is a risk of developing MA 10 years after diagnosis in patients with PF, it should be kept in mind that MA may also develop at an earlier period. Therefore, especially patients who are untreated and whose chronic inflammatory process continues in the fistula tract should be closely monitored for MA.
