Tract embolization with histoacryl and gel-foam after percutaneous hepatobiliary interventions in patients with ascites

Abstract

Background and aims: To determine the efficacy of percutaneous tract embolization with histoacryl and gel-foam in patients with ascites and coagulopathy after hepatobiliary interventions. Material and methods: Six tracts were embolized in five patients. Initially, a piece of absorbable gelatine sponge (Gel-foam) was deposited at the entrance point of the catheter in to the bile duct through a peel-away sheath and 70% N-butyl cyanoacrylate (NBCA) was then injected into the tract. Ascites fluid sampling was then performed under ultrasonographic guidance to evaluate the presence of bile and/or hemoperitoneum. Results: All of the patients tolerated the procedure well without complications and all procedures were technically successful. Ascites fluid evaluation did not reveal bile peritonitis or hemoperitoneum. Conclusions: It is concluded that in the presence of ascites and coagulopathy, tract embolization with histoacryl and gel-foam after percutaneous hepatobiliary intervention is a safe and effective method in the prevention of bleeding or bile leakage after removal of the catheter.

Assitli hastalarda perkütan hepatobiliyer girişimler sonrası parankimal traktın histoakril ve gel-foam ile embolizasyonu

Abstract

Giriş ve amaç: Bu çalışmanın amacı; assit ve koagülopati problemi olan hastalarda, hepatobiliyer girişimler sonrası trakt embolizasyonunda histoakril ve gel foam'ın etkinliğini saptamaktı. Gereç ve yöntem: Beş hastada 6 trakt embolize edildi. Önce yırtılabilen kılıf içerisinden, absorbe olabilen jelatin parçaları (gel-foam), kateterin safra yoluna girdiği düzeye ilerletildi. Daha sonra %70'lik n-bütil siyanoakrilat trakt içerisine enjekte edildi. İşlem sonrası assit sıvısından, safra varlığı ve hemoperitoneumun değerlendirilmesi amacıyla ultrasonografi eşliğinde örnek alındı. Bulgular: Tüm olgular işlemi kolaylıkla tolere etti ve işlem teknik olarak başarı ile gerçekleştirildi. Embolizasyon sonrası elde olunan assit sı- vısında safra veya kana ait bulgu saptanmadı. Sonuç: Assit ve koagülopati varlığında, perkütan hepatobiliyer girişimler sonrası kateterin çekilmesinden sonra ortaya çıkabilen kanama ve safra kaçağının önlenmesinde, histoakril ve gel-foam ile trakt embolizasyonu etkili ve güvenli bir yöntemdir.
