Arthroscopy in the diagnosis and treatment of mediopatellar plica syndrome of the knee

Abstract

27 patients with arthroscopicafly diagnosed and treated mediopatellar plica syndrome were reviewed average 16 months (6-33 months) after the operation. There were 22.vales and 5 females whose ages were average. 29 years (range, 16-63 years). A history of trauma to the same knee was present in 78% of the patients. The most frequent complaint was pain with activity, while ascending and descending stairs, and pain with prolonged flexion of the knee. Only 2 of the patients were thought to have hypertrophic mediopatellar pfica clinically. Arthroscopy revealed additionallesions in 44% of the patients. Arthroscopic segmental resection was performed in all cases. According to the Lysholm scoring scale, results were excellent in 13 (48%), good in 6 (22 %). fair in 3 (11%), and poor in 5 (19%). All the poor results were encountered in patients with additionallesions. 24 patients returned to their preinjury activity levels (Tegner). 2 patients have not tried their preinjury levels, and 1 patient has reduce his activity level. It is concluded that arthroscopy is an invaluable tool for the diagnosis and treatment of the plica syndrome of the knee.

Mediopatellar plika sendromunun tanı ve tedavisinde artroskopinin önemi

Abstract

Artroskopik olarak tanı konan ve tedavi edilen 27 mediopatellar plika sendromu olgusu ortalama 16 ay (633 ay) sonra değerlendirildi. Hastaların 22'si erkek, 5'i kadın olup, ortalama yaş 29 (16-63 yaş) idi. Olguların %78'inde aynı dilde travma öyküsü mevcuttu. En sık yakınma aktivite ile, merdiven inip çıkarken ve uzun süreli diz fleksiyonu ile ağrı idi. 27 hastanın 25'inde klinik olarak farklı tanılar konmuştu. Artroskopik muayenede olguların % 44'ünde ek lezyonlar saptandı. Tüm olgularda patolojik plika segmental olarak rezeke edildi. Lysholm skalasına göre 13 olgu (%48) mükemmel, 6 olgu (%22) iyi, 3 olgu (%11) orta, 5 olgu (%19) kötü olarak değerlendirildi. Kötü sonuç alınan olguların tümünde ek lezyonlar olduğu dikkati çekti. 27 hastadan 24'ünün yaralanma öncesi aktivite düzeylerine (Tegner) döndükleri görüldü. 2 hasta eski düzeyine dönememiş, 1 hasta ise aktivite skorunu düşürmüştü fakat buna razıydı. Plika sendromunun tanı ve tedavisinde artroskopinin son derece önemli olduğu sonucuna varıldı.
