Carpal tunnel syndrome (clinic and neurophysiological evaluation in the pre-and postoperative period)

Abstract

In this study, 27 hands of 23 patients with idiopathic carpal tunnel syndrome wich and gne under operative treatment were evaluted retrospectively according to their subjektive complaints, physical examinations and ENMG results in pre and post operative period. Follow-up period was 22.5 monts. In the preoperative period, paresthesia ratio was found as 92.6%, night pain 74.1%, weakness 51.9%, positive Phalen test 92.6%, pozitive Tinnel test 96.3%, thnar atrophy 33.3%, strength loss 33.3%, and sensorial loss was found as 55.5%. In all cases, ENMG examinations showed that, median motor latency was greater than 4.3 ms. In the postoperative period, paresthesia ratio was found as l4,8% weakness 11.1%, but none of the patients complanied abouth night pain. In physical examinatinos, positive Phalen test, positive Tinnel test, thenar atrophy, stength loss was found l4.8% and sensorial loss was fond as 11.1%, Ten hands (37%) showed pathological median motor latency. The ratio of emprovement of all subjective complaints was found as 85.2%, and also subjective complaints together with physical examinations and findings showed a ratio as 55.6%. Improvement difference between the subjective complaints, clinical and neurophysiological findings were discussed.

Karpal tünel sendromu (ameliyat öncesi ve sonrası klinik ve nörofizyolojik değerlendirme)

Abstract

Bu çalışmada idiopatik karpal tünel sendromu nedeniyle ameliyat edilen 23 olgunun 27 eli ameliyat öncesi ve sonrası subjektif yakınmaları, fizik muayene ve ENMG bulgularına göre retrospektif olarak değerlendirildi. Olgular ortalama 22.5 ay süreyle izlendi. Ameliyat öncesinde, parestezi %92.6 gece ağrısı %74.1, güçsüzlük %51.9, pozitif phalen testi %92.6, pozitif Tinnel testi %96.3 tenar atrofi %33.3, kuvvet kaybı %33.3, duyu kaybı %55.5, olarak bulundu. Olguların tümünde ENMG’den n.medianus distal motor latansı (DML) 4.3 ms’den uzun idi. Ameliyat sonrasında, parestezi % 14.8, güçsüzlük %11.1 olarak bulundu. Hiçbir olguda gece ağrısı mevcut değildi. Fizik muayenede pozitif Phalen testi, pozitif Tinnel testi, tenar atrofi ve kuvvet kaybı % 14.8, duyu kaybı %11.1 olarak bulundu. ENMG bulguları 10 (%37) elde patolojik sanırlardaydı. Subjektif yakınmaların düzelme oranı % 85.2, subjektif yakınmalar,
Fizik muayene ve ENMG bulgularının birlikte düzelme oranı %55.6 olarak bulundu. Subjektif yakınmalar, klinik ve nörofizyolojik bulgular arasındaki düzelme farklılıkları tartışıldı.
