Short segment pedicle screw instrumentation in surgical treatment of lumbar spine injuries

Abstract

Short segment pedicle screw instrumentation was performed on 6 cases who had spine injuries in the levels of L2-L4 at Orthopaedics and Traumatology Clinic in Haydarpaşa State Hospital, Istanbul. between August 1991 and May 1993. The technique was performed using ALICI instrumentation in 3, CD (Cotrel-Dubousset) in 2, and DIAPASON in one of the patients. The average age of the patients was 34.3 (27-42), and three were female. The mean follow-up was 14.5 months (6-33 months). Preoperative average local kyphosis angle was 2.6 degrees and 2.3 degrees in the last follow-up. Average local fracture angle was 13.8 degrees preoperatively and 4.3 degrees in the last follow-up. Average preoperatively compression of anterior vertebral body percentage was 34.6% and in the last follow-up it was 14.2%. As complications: in one case, screw was sent into the intervertebral disk space; in one case, screw was bent and in one anothar case, the upper screws were broken in the 8th and 12th months postoperatively. In the reductions obtained postoperatively, it was observed that, in local fracture angle and in local kyphosis angle there were 2.5 degrees and 10.3 degrees loss of correction. It is concluded that, in the injuries of lumbar spine which require surgical intervention, special laminar hook dependent short segment pedicle screw instrumentation systems can be utilised to restore lumbar lordosis and to leave mobile segments as much as possible.

Lomber omurga kırıklarının cerrahi tedavisinde kısa segment pediküler vida uygulaması

Abstract

Ağustos 1991-Mayıs l993 tarihleri arasında, İstanbul Haydarpaşa Numune Hastanesi Ortopedi ve Travmatoloji servisinde L2-L4 seviyelerinde omurga yaralanması olan 6 olgunun 3'üne ALICI, 2'sine CD (Cotrel Dubousset) ve 1'ine DIAPASON enstrumanları kullanılarak kısa segment pedikuler vida yöntemi uygulandı. 3'ü bayan olan olgularımızın ameliyat edildiklerindeki ortalama yaşları 34.3 (27-42) idi. Ortalama 14.5 ay (6-33 ay) takip edilen olgularımızda: ameliyat öncesi ortalama 2.6° olan lokal kifoz açısı son kontrolde ort. 2.3°; ameliyat öncesi ortalama 13.8° olan lokal kırık açısı son kontrolde ortalama 4.3°: ameliyat öncesi ortalama %34.6 olan omur cismi anterior kompresyon oranı son kontrolde ortalama %14.2 olarak saptandı. Komplikasyon olarak; 1 olguda vidanın disk aralığına girdiği, 1 olguda vida eğilmesi, 1 olguda ameliyattan sonraki 8. ve 12. aylarda üst iki vidanın kırıldığı saptandı. Ameliyat sonrası elde edilen redüksiyonlarda lokal kırık açısında 2.5° lokal kifoz açısında 10.3° korreksiyon kaybı olustuğu belirlendi. Bu bölgenin cerrahi tedavi gereken yaralanmalarında: mümkün olduğunca çok hareketli segment bırakarak, lomber lordozu restore edilecek şekilde eğim verilmiş çubuklarla özel laminer çengel destekli kısa segment pediküler vida yöntemlerinin uygulanabileceği sonucuna varıldı.
