16 patients with unstable fractures of the pelvis were treated using a new and simple trianguler anterior frame external fixator which was designed in our clinic. According to Tile classification, there were 6 type B-1, 3 type B-3, 5 type C-1 and 2 type C-3 injuries. Since two patients died Iater, 14 patients could be evaluated. The patients were followed for an average of 16. 7 months following injury. The clinically results were satisfactory in 12 patients (85. 7%) and unsatisfactory in 2 patients (14.3%). Biomechanical tests of the fixator (Selçuk type pelvic external fixator, SPEF) were carried out on cadaveric pelves. The clinical and biomechanical results suggested that the SPEF is readily applicable and safe. Therefore, we conclude that this method may be preferable for most of unstable pelvic ring injuries.
Stabil olmayan pelvis yaralanması olan 16 hasta, kliniğimizde geliştirilen yeni ve basit bir trianguler anterior eksternal fiksatör ile tedavi edildi. Tile sınıflandırmasına göre; 6 hasta B-1, 3 hasta B-3, 5 hasta C-l, 2 hasta C-3 olarak değerlendirildi. iki hasta öldüğü için 14 hasta değerlendirmeye alındı. Hastalar yaralanmadan sonra ortalama 16.7 ay takip edildiler. Klinik olarak 12 hasta (%85.7) başarılı, 2 hasta (% 14.3) başarısız sonuç şeklinde değerlendirildi. Fiksatörün (Selçuk tipi pelvis eksternal fiksatör, SPEF) biyomekanik testleri kadavra pelvisleri kullanılarak gerçekleştirildi. Klinik ve biyomekanik bulgular SPEF'ün kolay uygulanabilen ve emniyetli bir tedavi yaklaşımı olduğunu göstermiştir. Biz, SPEF'nün çoğu anstabil pelvis yaralanmasının tedavisinde tercih edilebilecek bir tedavi yaklaşımı olabileceği kanaatindeyiz.
