The treatment of lower extremity pseudoarthrosis with Ilizarov method

Abstract

In our clinic, 30 cases of lower extremity pseudoarthrosis were treated with Ilizarov method between 1990 - 1998. The localizations of pseudoarthrosis were tibia in 14 cases, femur in 11, femur and tibia in one, tibia bilaterally in four. The reasons of pseudoarthrosis were fracture in 22 cases, osteomyelitis in 3, tumor resection in 3, and congenital pseudoarthrosis of tibia in two. The cases classified as B1 in 8 cases, B2 in 10, B3 in 8, A1 in 3, A2-1 in one, according to Paley’s classification. We used monofocal technique in 11 cases, bifocal in 16, trifocal in 2, and tetrafocal in one. Mean bone loss was 6.6 cm (0-29). Results were appraised according to Paley’s functional and radiological criteria. The radiological results were excellent in 19 cases, good in 8, fair in one, and poor in two. The functional results were excellent in 11 cases, good in 18, and fair in one. 19 patients had active osteomyelitis before operation and osteomyelitis was healed in 16 patients. We suggest that Ilizarov method is the best choice in the treatment of lower extremity pseudoarthrosis especially when complicated with infection.

Abstract

Alt ekstremite psödoartrozları üst ekstremitelerden farklı olarak hastanın immobilizasyonuna neden olmaktadır. 1990-1998 yılları arasında İlizarov metodu ile 30 alt ekstremite psödoartrozlu olgu tedavi edildi. Psödoartroz 14 tibia, 11 femur, 1 olguda femur ve tibiaya, 4 olguda bilateral tibiaya yerleşimli idi. Psödoartroz nedenleri; kırık (22), osteomyelit (3), tümör rezeksiyonu (3), konjenital tibia psödoartrozu (2) idi. Ameliyat öncesi olgular Paley'in psödoartroz sınıflamasına göre 8 Bl, 10 B2, 8 B3, 3 Al, l A2-1 tip psödoartroz olarak değerlendirildi. Onbir monofokal, 16 bifokal, 2 trifokal, 1 tetrafokal tedavi uygulandı. Kemik kaybı ortalama 6,6 cm (0-29) idi. Sonuçlar Paley'in radyolojik kriterlerine göre; 19 mükemmel, 8 iyi, 1 orta, 2 kötü, fonksiyonel kriterlerine göre ise; 11 mükemmel, 18 iyi, 1 orta olarak degerlendirildi. Tedavi öncesi aktif infeksiyon bulunan 19 olgunun 16 tanesinde infeksiyon iyileşti. Alt ekstremitelerin özellikle infekte ve defektif psödoartrozlarında İlizarov yönteminin en iyisi olduğunu düşünmekteyiz.
