Rice bodies of synovial origin in the knee: a case report

Abstract

Rice bodies are free corpuscles of synovial origin with a cartilage-like appearance and may reach hundreds in number within the intraarticular space. Rheumatologic or infectious lesions that may cause synovial hypertrophy play major role in the etiology. Although this entity is recognized by rheumatologists, it is rarely mentioned in the orthopedic literature. Hundreds of bodies in the form and size of rice bodies were noted in the left intraarticular space of an 11-year-old male patient who underwent diagnostic arthroscopic drainage and partial synovectomy. Pathologic examination showed that some of the bodies consisted only of fibrin, with some having a collagenous nucleus within the fibrin. No evidence of recurrence was found during his orthopedic follow-up of 2.5 years, no underlying rheumatologic pathology was detected, as well.

Abstract

Pirinç tanesi cisimcikleri (Rice bodies) eklem içinde yüzlerce sayıda, kıkırdak taneleri gibi görünen, sinovya kaynaklı serbest cisimlerdir. Etiyolojide genellikle sinovyal hipertrofiye sebep olan romatolojik veya enfeksiyöz patoloji mevcuttur. Patoloji, romatologlar tarafından bilinmekle beraber, ortopedik literatürde çok az yer almıştır. Sol dizde sürekli sinovit nedeniyle tanısal amaçlı artroskopik biyopsi uygulanan 11 yaşında erkek hastanın eklem boşluğunda yüzlerce sayıda, pirinç tanesi şekil ve büyüklüğünde cisimcikler saptandı ve artroskopik drenaj ve parsiyel sinovektomi uygulandı. Patolojik incelemede bir kısım cisimciklerin sadece fibrinden oluştuğu, bir kısmının ise fibrin içinde kollajen bir çekirdek içerdiği gözlendi. Hastanın 2.5 yıllık ortopedik klinik takibinde nüks gözlenmedi; romatolojik takibinde de anlamlı patolojik bulgu saptanmadı.
