Sakroiliak kompleksin instabilitelerinde S1-pediküloiliak fiksasyon

Abstract

Sakroiliak eklemin fiksasyonunda günümüzde plak vida ve rodlardan oluşan farklı teknikler kullanılmaktadır. Bu yöntemler ile çoğu zaman ancak anatomiğe yakın redüksiyon elde edilebilmektedir. S l'in pedikülü ile iliak kemiğin arasinda yapılacak bir fiksasyon ise hem redüksiyonu anatomik olarak sağlamada, hem de bunu korumada önemli bir avantaj sağlamaktadır. Sakroiliak kompleksin instabilitelerinde SI'in tek pedikülü ile iliak kanat arasinda fiksasyon bugüne kadar uygulanmamış bir tekniktir. Bu yöntemle hem sakroiliak dislokasyonlar, hem de alar zondaki vertikal sakrum kırıklarınm redüksiyonu ve stabilizasyonu mümkündür. Burada S l pedikül vidası ile iliak vida ve bunları birleştirici parçadan oluşan konstrüksiyon uyguladığımız sakroiliak instabiliteli iki olgu ve cerrahi teknik sunulmaktadır. Her iki olguda da komplikasyonsuz iyileşme elde edilmiştir.

Abstract

Stabilization of the sacroiliac joint may be achieved through various plate-screw and rod fixation techniques; however, these techniques rarely allow appropriate anatomic reduction of the joint. We describe a new technique for the treatment of sacroiliac joint complex instabilities, using a rod-screw fixation between a pedicle of the S1 vertebra and the iliac bone, and providing the advantages of an easily achieved and maintained anatomic reduction. Sacroiliac dislocations and vertical sacrum fractures in the alar zone may thus easily be reduced and stabilized. We hereby report two patients with sacroiliac instability treated with this new construction consisting of an S1 pedicle screw, an iliac screw, and a connecting rod, with special emphasis on the surgical technique. Both patients showed complete healing without any complications.
