A case of synovial hemangioma

Abstract

Synovial hemangioma is a rare lesion that most commonly involves the anterior compartment of the knee. It usually occurs in adolescents and presents as recurrent episodes of swollen knee and pain. A sixteen-year old female patient presented with complaints of swelling and pain in her left knee. Physical examination showed a painful mass in the inferolateral part of the patella. Plain radiographs were normal. Magnetic resonance imaging showed a lobular mass extending from the lower part of the patella to the infrapatellar fat pad. Pathologic examination of the excised specimen revealed proliferated vascular structures of varying caliber arising below the hypertrophied synovial tissue and extending to the deep layers. A diagnosis of synovial hemangioma was made. Following total excision, prognosis is usually excellent and recurrence is rare. No recurrences were detected during a follow-up period of 16 months.

Sinovyal hemanjiyom

Abstract

Sinovyal hemanjiyom sıklıkla diz ekleminin anterior kompartmanını tutan, nadir rastlanan bir lezyondur. Genellikle adölesan yaşta görülür; alevlenme ve remisyonlarla seyreder ve dizde şişlik ve ağrı şikayetiyle ortaya çıkar. On altı yaşında kadın hasta sol dizinde şişlik ve ağrı şikayetiyle başvurdu. Fizik muayenede patellanın inferolateralinde ağrılı, orta sertlikte kitle saptandı. Düz grafilerinde özellik görülmedi. Manyetik rezonans görüntülemede patellanın altından infrapatellar yağlı doku içine uzanan lobüler yapıda kitle gözlendi. Eksize edilen kitlenin histolojik incelenmesinde hipertrofik sinovyumun altından başlayarak derin tabakalara uzanan çeşitli büyüklükte prolifere damar yapıları izlenerek sinovyal hemanjiyom tanısı kondu. Sinovyal hemanjiyomun total eksizyonundan sonra prognoz çok iyi olup nüks oranı oldukça düşüktür. Hastanın 16 aylık takip sürecinde nüks saptanmadı.
