The treatment of severe scoliotic curves

Abstract

The progressive curves over 100 degrees is commonly seen if the early onset progressive curves could not be treated effectively. Cardiorespiratory failure, back pain, neurological complications and the apparence of bad cosmesis are commonly seen in progressive curves. The complications in the treatment of the progressive curves are high. Correction should be obtained by halofemoral traction preoperatively slowlyand gradually with careful neurologic monitoring, then Harrington instrumentation and posterir fusian should be done without further correction. If the correction is obtained by Harrington instrumentation alone intraoperative spinal cord monitoring or wakeup test should be done. In thes article 17 scoliotic cases, already over 100 degrees which were treated surgically in the department of Orthopedic and Traumatologic Surgery of Ankara University Medical faculty between 1978-1982 are presented. Infection was seen in 2 patients and pseudoarthrosis in 3 patients. Our complication rate was %31.25.

Abstract

100 derecenin üzerindeki ileri skolyozlar, erken yaşta başlayan ilerleyici skolyozların etkin bir şekilde tedavi edilememesiyle oluşur. İleri derecedeki skolyozlu hastalarda solunum ve kalb yetmezliği, sırt ve bel ağrıları, nörolojik komplikasyonlar ve kozmetik bozukluklar kaçınılmaz bulgulardır. İleri derecedeki skolyozların tedavisinde komplikasyon oranı fazladır. Skolyozun preoperatif halo-femoral iskelet fraksiyonu ile kontrollü bir şekilde düzeltilmesi ve sonra Harrington instrumentasyonu uygulanması; ya da doğrudan Harrington instrumentasyonu yapılacaksa ameliyatta uyandırma testi ya da spinal moniterizasyon gereklidir. Bu makalede 1978-1982 yılları arasında Ankara Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Ana Bilim Daında, ameliyatla tedavi edilen 100 derecenin üzerinde 17 skolyozlu hasta inceledi. 2 hastamızda enfeksiyon, 3 hastamızda psödoartroz görüldü. Komplikasyon oranımız % 31.25 olarak saptandı.
