İnternal tespit sonrası femur boynunun kısalması

Abstract

Amaç: Bu çalışmanın amacı femur boyun kırıklarının internal tespiti sonrası femur boynunun kısalmasına neden olan faktörlerin değerlendirilmesi idi.
Çalışma planı: Femur boyun kırığı olan 86 hasta Mayıs 2004 ila Ocak 2011 tarihleri arasında üç paralel kanüllü vida kullanılarak tedavi edildi. TraumaCad yazılımı kullanılarak, vida çapı ile düzeltilen anteroposterior grafilerde, yatay (X), dikey (Y) ve de bileşke (Z) vektörleri boyunca (XÆ, YÆ, Z) femur boynunun kısalması ölçüldü. Bunun yanı sıra, yaş, cinsiyet, Garden sınıflaması, Garden dizilim indeksi, Pauwels açısı, Singh indeksi, vücut kitle endeksi ve ağırlık verme süresi de analiz edildi.
Bulgular: Sekiz ile 36 ay arası süren takipleri boyunca, 86 hastanın 33’ünde (%38.3) abdüktör kaldıraç kolunda belirgin derecede (5 mm’den daha fazla) femur boynu kısalması söz konusuydu. Ortalama Harris skoru (HSS) 90.05±7.04 (dağılım: 71-100) olarak kaydedildi. Çok değişkenli lojistik regresyon modelinde 5 mm’den daha büyük kısalmalardaki 5 belirleyici faktör; yaş, Singh indeksi, Pauwels sınıflaması, Garden dizilim indeksi ve vücut kitle indeksi olarak bulundu.
Çıkarımlar: Femur boyun kırıklarının tedavisinde kullanılan üç parallel kanüllü vida kullanımına bağlı femur boynu kısalması sıklıkla görülen bir olaydır. İnternal tespit sonrası gözlemlenen femur boynu kısalması birçok kofaktöre bağlı olabilir

Femoral neck shortening after internal fixation

Abstract

Objective: The aim of this study was to assess the factors affecting femoral neck shortening after internal fixation of femoral neck fractures.
Methods: Eighty-six patients with femoral neck fractures were treated using three parallel cannulated screws between May 2004 and January 2011. The shortening of the femoral neck in the horizontal (X), vertical (Y), and along the resultant along the (Z) vector (X→, Y→, Z→) was measured on anteroposterior radiographs corrected by screw diameter and analyzed using TraumaCad software. Age, gender, Garden classification, Garden’s alignment index, Pauwels angle, Singh index, body mass index and weight-bearing time were also analyzed.
Results: Follow-up duration was 8 to 36 months. Significant femoral neck shortening of the abductor lever arm (greater than 5 mm) was present in 33 of 86 (38.4%) patients. Average Harris score
(HSS) was 90.05±7.04 (range: 71 to 100). The 5 predictors for shortening greater than 5 mm in the multivariate logistic regression model were age, Singh index, Pauwels classification, Garden’s alignment index and body mass index.
Conclusion: Femoral neck shortening associated with three parallel cannulated screws for fixation of femoral neck fractures is a common phenomenon. Femoral neck shortening after internal fixation is affected by multiple cofactors.
