Ocak 1966 Ağustos 1975 yılları arasında kliniğimizde tromboflebittanısı alarak tedavi gören 315 vaka içerisinde 11 üstekstremite derin ven trombozu (% 3,46) tesbit edilmiştir. 11 vakadan2'si kadın, 9'u erkekti. Vakalardan 7'sinde olay solda,2 vakada sağda, 2 vakada ise bilateraldi. Etyopatogenetik faktörolarak 7 vakanın aşırı effor sonrası olayın gelişmesi effortrombozu terimini doğrulamaktadır.Hastalığın tanısında dikkatli bir anamnez, fizik muayeneyanında, fleborafi yanılgıya yol açmadan kesin tanıya ulaştırır.Ven trombozu ile birlikte arteriel spasm olabileceği ve akut artertıkanması izlenimi verebileceği akılda tutulmalıdır. Hiçbirvakada üst ekstremite akut derin ven trombozuna bağlı pulmoneremboli görülmemiştir.Hastalığın tedavisinde antikoagülan tedavi, yatak istirahative ilgili uzvun kalp seviyesi üstünde elevasyonu başarılı sonuçlarvermektedir. Tedavide önerilen venöz trombektomi ancakilk 48 - 72 saatlik yeni vakalarda başarılı olabilir. Geç vakalardatrombüsün organize olması işlemi güçleştirmesi yanında, retromboz,ve pulmoner emboli olasılığını artırır. Bunun yanındaböyle bir işlem ancak tecrübeli hekimler tarafından uygulanmalıdır.

Between January and August 1975, 315 trombophlebitiswere diagnosed and treated in our clinic. Among these, 15 casesof primary acute deep vein thrombosis of the upper extremitywere identified (% 3,46). 2 of the 11 patients were female and 9 men. In these patients thrombosis was on the left in 7, on the right in 2, and bilateral in the other 2 cases. It was found that the onset was expressed after extensive muscle excercise, Which confirms the patogenetic effort trombosis concept (63,6).Carefull questioning, physical examination and phlebographycan prove the correct diagnosis. One must kept in mindthat arteriel spasm may complicate the clinical picture and maylead to a false acute arterial obstruction disition. None of this11 cases showed any signs of pulmonary embolism.In the treatment of the process, bed rest, anticoagulanttherapy and elevation of the extremity gives succesfull results.Venous thrombectomy can only be indicated and may give succesfullresults in the first 48 - 79 hours. In late cases because ofthe organisation and recanalisation process of the thrombi,thrombectomy may porvoke pulmonary embolism and rethremhbosis.
