Kliniğimizde 4'ü böbrek biyopsisiyle, biri de otopsiyle tanı konulan 5 diabetik glomerulosklerozlu vak'a takdim edilmiştir. Hastalardan ikisinin 40 yaşın çok altında olmasına, nefrotik sendrom, hipertansiyon ve proteinüri triadının yalnız bir vak'ada görüldüğüne işaret edilmiştir. Az veya çok proteinürisi olan her diabetli hastaya böbrek biopsisiyle erken tanıyı önerdik. 5 vak'anın histopatolojik incelenmesinde, 3 vak'ada diabetik glomeruloskleroz için spesifik olan bazal membran kalınlaşması, atffereni ve efferent arteriollerde hyalin birikmesi ve mesangiumda hyalin nodülleri tesbit edilmiştir. Diğer vak'alarımızda diffüz bazal membran kalınlaşması ve arteriollerde hyalin depolanmaları görülmüştür.

Four cases of diabetic glomerulosclerosis, diagnosed three by kidney biopsy and one by autopsy, were presented in our clinic. It was noted that two of the patients were well under 40 years of age, and the triad of nephrotic syndrome, hypertension, and proteinuria was observed in only one case. We recommended early diagnosis with kidney biopsy for all diabetic patients with mild or severe proteinuria. Histopathological examination of the five cases revealed basement membrane thickening, atherosclerosis, hyaline deposition in efferent arterioles, and hyaline nodules in the mesangium, all of which are characteristic of diabetic glomerulosclerosis. In the remaining cases, diffuse basement membrane thickening and hyaline deposition in arterioles were observed.
