Our understanding of the pathology of the suprahepatic veins has only begun to increase in recent years. The basis of the pathology is the narrowing and compression of these veins. Etiologically, intrahepatic and extrahepatic diseases, as well as lesions of the suprahepatic and inferior vena cava, are important. Pathophysiologically, portal hypertension and compensatory hyperplasia in the liver resulting from obstruction of the suprahepatic veins are noteworthy. Clinical symptoms can be acute or chronic, depending on the nature and location of the obstruction. Diagnostic evaluation requires splenoportography, hepatography, and cavography, measurement of pressures in these systems, and liver aspiration biopsies. Treatment can be medical or surgical, depending on the case. The results are quite encouraging.
Vena supra-hepatica'ların patolojisi hakkındaki bilgilerimiz ancak son yıllarda artmaya başlamışdır. Patolojinin esasını bu venaların daralma ve sıkışması teşkil eder. Etyoloji bakımdan karaciğer içi ve dışı hastalıklar, vena supra-hepatica ve vena cava inferior'un lezyonları önemlidir. Fizyopatoloji yönünden vena supra-hepatica'ların tıkanması sonucu portal hipertansiyon ve karaciğerde kompensatris hiperplazi dikkati çeker. Klinik belirtiler ukanmanın teşekkülüne ve lokalizasyonuna bağlı olarak acut ve kronik bir şekilde olmaktadır. 'Teşhis yönünden spleno-portografi, hepatografi ve kavografi, bu sistemlerin basınçlarının ölçülmesi ve karaciğer aspirasyon biopsilerinin yapılması lüzumludur. Tedavi, vak'asına göre, medikal ve şirürjikal olabilir. Sonuçlar oldukça cesaret vericidir.
