A.M.P.E. is a serious disease and its diagnosis and treatment are extremely difficult. During the last five years we have diagnosed three cases but we haven't had the change to operate them. The conclusion we reached is that in order to decrease the Mortalıty rate at the M.P.E. supporting Bypass or total By-pase should be ready to be used for the operation.

Masiv pulmoner embolizm zamanımızda en fazla üzerinde durulan hastalıklardan biri olup gerek teşhis ve gerekse tedavisindeki zorlük sebebiyle üzerinde yapılan araştırmalar gittikçe çoğalmaktadır. Kliniğimizde teşhis edildiği halde süratle ölüme giden 3 vaka incelenmis ve maalesef bunlara gerekli cerrahî tedavi yapılabilmesi için zaman bulunamamıştır. Ancak vakalarımızdan bir tanesinin masiv pulmoner emboli olmasına rağmen uzun seneler yasamış olması pulmoner embolizmde cerrahî tedaviye gitmeden önce konservatif metodlarında denenmesi mecburiyetini ortaya koyacak niteliktedir. Hastalığın tedavisinin cerrahî olduğu üzerinde büyük bir çoğunlukla durulmasına rağmen ameliyat sonu neticelerin pek yüz güldürücü olmaması araştırıcıları hastalıktan korunulması için gereken işlemlerin yapılması üzerine çekmiştir. Bugün acil cerrahi tedavi yanında konservatif tedavinin daha iyi neticeler verdiği üzerinde geniş münakaşalar yapılmaktadır. Ancak cerrahî müdahale şekillerinin değişmesi ve bozulan kardiyorespiratuvar sistemin ekstrakorporeal sistem aracılığı ile kısmen dahi olsa düzeltilmesi yeniden cerrahî tedavi yollarını ön plâna getirmiştir. Modern kliniklerde süratle kullanılmaya müsait portatif pompa cihazlarının tatbike konulmasından sonradır ki, ameliyat mortalitesi büyük bir hızla düşmeye başlamıştır.
