<p>This clinical series includes 41 cases of solitary pyogenic liver abscess treated and reviewed between 1956–1971. The abscesses were located in the right hepatic lobe in 82% of patients and predominantly in the superior dome region. Abscess volumes ranged from less than 1 liter to over 5 liters in some cases. Cultures were sterile in 45% of aspirates, consistent with previous reports. Most patients presented with fever, right upper quadrant pain, and hepatomegaly; leukocytosis and accelerated sedimentation rates supported inflammatory diagnosis. Serious complications occurred in 12 patients, including diaphragmatic rupture, pleural perforation, biliary fistula, and peritoneal spread. Acute liver failure developed in 7 patients—all fatal. Surgical drainage was performed via anterior abdominal route in all cases. Mortality rate was 26.8%, mostly due to hepatic insufficiency.</p></trans-abstract>
