                            <p>This retrospective study assesses the results of 46 patients treated via a modified Harkins procedure—selective gastric vagotomy, hemigastrectomy, and gastroduodenostomy. Patients were diagnosed and operated exclusively by Prof. Bumin. The preoperative average free gastric acid was 35 mEq/L, and postoperative levels averaged 8.1 mEq/L, with 42% of cases recording 0 mEq/L. No operative mortality was reported. Only one patient showed postoperative peritonitis symptoms, managed conservatively. Follow-up revealed consistent symptom resolution, weight gain (average 4.2 kg), and absence of recurrent ulcer, anemia, or dumping syndrome. The procedure included anatomical refinements such as oblique duodenal incisions parallel to vasa recta in 4 cases to improve physiological outcomes. The discussion emphasizes the need for selective vagotomy combined with distal resection to avoid residual acid production and post-vagotomy complications.</p></trans-abstract>
                                                                                                                                    <abstract><p>Bu çalışmada Prof. Dr. Orhan Bumin tarafından Ankara Üniversitesi I. Şirürji Kliniği’nde 46 hastaya uygulanan modifiye Harkins ameliyatının sonuçları değerlendirilmiştir. Tüm hastalara seçici gastrik vagotomi, hemigastrektomi ve gastroduodenostomi uygulanmıştır. Operasyon öncesi mide serbest asit ortalaması 35 mEq/L iken; sonrası ortalama 8.1 mEq/L’ye düşmüş, hastaların %42’sinde asit 0 mEq/L bulunmuştur. Ameliyat sonrası mortalite olmamıştır. Sadece bir hastada postoperatif peritonit bulguları görülmüş, konservatif yaklaşımla iyileştirilmiştir. Uzun dönem takiplerde ülser nüksü, anemi veya dumping sendromu izlenmemiş; ortalama 4.2 kg kilo alımı gözlenmiştir. Son 4 vakada, gastroduodenostomi için duodenum kesisi vasa recta’ya paralel gerçekleştirilmiş ve daha fizyolojik sonuçlar elde edilmiştir. Çalışma seçici vagotominin, antrum rezeksiyonu ile birlikte uygulanmasının nüks oranlarını minimize ettiğini ve postvagotomi komplikasyonlarını önlediğini tartışmaktadır.</p></abstract>
