Bu teknik yazıda Prof. Dr. Bumin'in modifiye Harkins yöntemi kapsamındaki seçici gastrik vagotomi, hemigastrektomi ve gastroduodenostomi uygulamasının detayları aktarılmaktadır. Vagotomi'nin, duodenum kesildikten sonra uygulanması sayesinde mide sinir dallarının daha net görülmesi sağlanmıştır. Küçük kurvatür, içten bir sıra tek tek ipek dikişle kapatılmış; böylece nedbe dokusu minimal tutulmuş ve mide işlevleri erken dönemde normale dönmüştür. Gastroduodenostomi bölgesi, pankreas başı üzerine sabitlenerek sızıntı riski azaltılmış, dumping sendromu önlenmiştir.
