                            <p>This report presents a rare case of brucellosis with central nervous system involvement in a 39-year-old male farmer from Samsun. The patient exhibited intermittent fever, night sweats, and mild nuchal rigidity. Blood culture yielded Brucella melitensis, and lumbar puncture showed 100 leukocytes/mm³ in clear CSF with predominantly lymphocytes, confirming concurrent meningitis. CSF culture also grew Br. melitensis. Both isolates were identified as biotype 2 via FAO/WHO Brucella Reference Center using biochemical and bacteriostatic panels. The organism showed sensitivity to ampicillin, oxytetracycline, and streptomycin. A combination therapy of streptomycin, terramycin, and sulfonamides led to full clinical and serological recovery. This represents one of the few documented cases with double isolation from blood and CSF, reinforcing the importance of evaluating neurological symptoms—even when mild—in brucellosis.</p></trans-abstract>
                                                                                                                                    <abstract><p>Bu çalışmada, Samsunlu 39 yaşında çiftçi bir erkek hastada görülen ve BOS (beyin omurilik sıvısı) ile kan örneklerinden Brucella melitensis biyotip 2 izole edilen nadir menenjit komplikasyonlu bruselloz olgusu sunulmuştur. Hastada ateş, terleme, baş ve boyun ağrısı gözlenmiş; BOS sıvısı berrak olup mm³’te 100 lenfosit içermiştir. Aynı anda hem kan hem BOS kültürlerinden Br. melitensis üretildiği teyit edilmiştir. FAO/WHO Brucella Referans Merkezi tarafından biyokimyasal ve serolojik testlerle biyotip 2 olarak tanımlanan mikroorganizma streptomisin, oksitetrasiklin ve ampisiline duyarlı bulunmuştur. Streptomisin–terramycin–sülfamit üçlü kombinasyonuyla yapılan tedavi hastada tam klinik düzelme sağlamıştır. Bu vaka, hem kültürel hem serolojik olarak merkezi sinir sistemi tutulumunun belgelenmiş nadir örneklerindendir ve enfeksiyon hastalıklarında menenjit belirtilerinin dikkatle değerlendirilmesi gerektiğini göstermektedir.</p></abstract>
