This paper presents six surgically observed cases of acute renal failure attributed to three hemolytic transfusion reactions, one post-hepatic insufficiency (hepatorenal syndrome), and two post-traumatic hypovolemic shocks. Cases were analyzed with respect to predisposing factors (prolonged oliguria, hypotension, sepsis), pathogenesis (arteriolar spasm, tubular obstruction, ischemia), and clinical phases (oliguric, polyuric, and recovery). The author discusses histopathologic findings such as distal tubular necrosis, interstitial edema, glomerular preservation, and hemoglobin-based obstruction. Management strategies are reviewed in depth: cautious fluid replacement, monitoring central venous pressure, hypertonic glucose with insulin for hyperkalemia, bicarbonate for metabolic acidosis, and mannitol/furosemide for diuresis induction. Preventive recommendations include avoiding vasopressors in shock therapy, ensuring adequate diuresis preoperatively in jaundiced or high-risk patients, and recognizing subclinical renal damage in aged individuals.
Bu çalışmada, cerrahi pratiğe ait toplam altı akut renal yetmezlik vakası sunulmaktadır. Üç vaka transfüzyon kaynaklı hemoliz, bir vaka hepatik yetersizlik (hepatorenal sendrom), iki vaka ise travma sonrası hipovolemik şok nedeni ile gelişmiştir. Oligürinin uzaması, hipotansiyon, sepsis gibi predispozan faktörler; arteriol spazmı, tubulus tıkanıklığı, iskemik süreçler gibi patogenezik mekanizmalar; ve oligurik, poliürik, iyileşme evreleri incelenmiştir. Histopatolojik olarak distal tubuluslarda nekroz, interstisyel ödem, glomerüler korunma ve hemoglobin kaynaklı tıkaçlar değerlendirilmiştir. Tedavi önerileri arasında dikkatli sıvı replasmanı, santral venöz basınç izlemine dayalı yönetim, hiperkalemi için insülinle birlikte hipertonik glikoz ve bikarbonat tedavisi, asidoz tamponlaması ve mannitol/furosemid ile diürez indüksiyonu yer almaktadır. Koruyucu strateji olarak; şok tedavisinde vazopressörlerden kaçınılması, sarılıklı veya riskli hastalarda ameliyat öncesi yeterli diürezin sağlanması, yaşlı bireylerde subklinik böbrek hasarının fark edilmesi vurgulanmaktadır.
