                            <p>This seminar discusses fluid and electrolyte loss following gastrointestinal surgeries, emphasizing postoperative homeostasis and organ-level response. It outlines total body water distribution, delineating intra- and extracellular compartments (42 liters in a 70 kg adult). Extrarenal losses (via gastrointestinal secretions, surgical drains, and third space sequestration) and renal losses are quantified. Notably, sodium is well-conserved unless depleted, whereas potassium is continuously lost even in deficit. Tables detail electrolyte concentrations across physiological and pathological states. A compartment-based replacement strategy is proposed, prioritizing sodium and potassium repletion timing, and caution in overhydration. Specialized replacement solutions for acid-base imbalance (e.g., bicarbonate, lactate, ammonium chloride-based infusions) are also recommended.</p></trans-abstract>
                                                                                                                                    <abstract><p>Bu seminer metni, gastrointestinal cerrahi sonrası karşılaşılan sıvı ve elektrolit kayıplarını irdeleyerek homeostazın korunması gerekliliğini vurgular. 70 kg bir erişkinde toplam vücut suyu dağılımı 42 litre olup bunun %60’ı intrasellüler, %40’ı ekstrasellüler bölmelerdedir. Renal ve ekstrarenal sıvı kayıpları ayrı ayrı değerlendirilmekte; mide aspirasyonları, safra ve pankreas fistülleri, barsak drenajları gibi nedenlerle ortaya çıkan ekstrarenal kayıplar hesaplanarak telafi ilkeleri sunulmaktadır. Sodyum eksiklikleri genelde iyi tolere edilmekteyken potasyum eksikliğinde vücut devamlı kayıp yaşar. Makalede çeşitli fizyolojik ve patolojik durumlara göre elektrolit konsantrasyonlarını gösteren tablolar ve çözümlemeler sunulmuştur. Dehidratasyonu önlemede aşırı sıvı yüklenmesi riskine karşı uyarılarda bulunulmakta; asit-baz dengesizlikleri için özel çözeltiler (bikarbonat, laktat, amonyum klorür bazlı infüzyonlar) önerilmektedir.</p></abstract>
