                            <p>The paper discusses the pathophysiology of duodenal ulcer, emphasizing the role of acid secretion, vagal stimulation, and gastrin production. Historical progression from Billroth I/II techniques through vagotomy (total, selective, and highly selective) and combined procedures are outlined. The author highlights how modern approaches (e.g., selective gastric vagotomy + antrectomy + Billroth I reconstruction) offer physiologically sound alternatives with lower recurrence rates. Surgical outcomes, dumping syndrome, post-vagotomy diarrhea, and complications are reviewed. The study supports preserving antral motility while minimizing acid load through targeted resections and nerve sparing strategies.</p></trans-abstract>
                                                                                                                                    <abstract><p>Bu derleme, duodenum ülserinin fizyopatolojisini ele alarak mide asiditesi, vagus sinir uyarımı ve gastrin üretiminin etkilerini vurgular. Cerrahi uygulamaların tarihsel gelişimi Billroth I/II yöntemlerinden total/selektif/yüksek selektif vagotomi ve kombine ameliyatlara kadar incelenir. Modern yöntemlerin (örneğin selektif mide vagotomisi + antrektomi + Billroth I rekonstrüksiyonu) fizyolojik açıdan üstün ve nüks oranı düşük alternatifler sunduğu gösterilmiştir. Ameliyat sonrası döküntü sendromu, vagotomi sonrası diyare ve diğer komplikasyonlar detaylandırılır. Çalışma, antrum motilitesini koruyarak asit yükünü azaltmanın cerrahide ideal yaklaşım olduğunu savunur.</p></abstract>
