Metoklopramid, bulantı şikâyeti ile başvuran hastalara sıklıkla reçete edilebilen, dopamin reseptör antagonisti olan bir ilaçtır. Akut distonik reaksiyonlar, metoklopramidin nadir rastlanan bir yan etkisidir. Bu makalede, tedavi dozunda metoklopramid kullanmakta iken, akut distonik reaksiyon gelişen bir olgu sunulmuştur. Olgumuzda tanı, öykü ile konuldu. Antikolinerjik bir ilaç olan biperiden HCL kullanılması ile şikâyetleri hızlıca ortadan kayboldu. Akut distonik reaksiyonlar nadiren faringeal ve laringeal kasların spazmı sonucu solunum sıkıntısına neden olabilir. Bu yüzden acil tedavi edilmesi gereken bir durumdur. Akut distoni şikâyeti ile başvuran hastalarda metoklopramid kullanımı akla gelmelidir.
Metoclopramide is a dopamine receptor antagonist, which can be prescribed frequently to patients, presenting with complaints of nausea. Acute dystonic reactions are rare side effects of metoclopramide. In this article, a case was presented with acute dystonic reactions while using therapeutic dose of metoclopramide. In our case, the diagnosis was based on history. With the use of an anticholinergic drug, biperiden HCL, complaints resolved quickly. Rarely, acute dystonic reactions can cause respiratory distress due to pharyngeal and laryngeal muscle spasm. Therefore, an emergency is needed to treat. The use of metoclopramide should be considered in patients presenting with complaints of acute dystonia.
