THE EFFECTS OF ENGLAND PUBLIC-PRIVATE PARTNERSHIP MODEL ON THE CITY HOSPITAL PRACTISES IN TURKEY: AN ANALYSIS IN THE CONTEXT OF POLICY TRANSFER

Abstract

In the health sector, the Public-Private Partnership (PPP) model was introduced in the UK in the 1990s and spread from this country to the world. Some of the countries implementing the PPP model have directly transferred the example of England and others have preferred to adapt to their own health systems. In this context, PPP is implemented in the health sector as three sub-models: “infrastructure-based model”, “discrete clinical services model” and “integrated PPP model”.  The aim of this study is to examine the city hospitals model that performed by based on the example of England PPP, in the context of policy transfer. For this purpose, the PPP model applied in city hospitals is compared with the UK PPP model in various aspects. Comparison is covered under the headings of the structure of private sector entrepreneurs, the sharing of duties and responsibilities of the public and private sector, the fees paid to the private sector, the establishment and function of the special purpose company, the sectors in which the model is applied, encountered risks and sharing of risks, the criticisms directed to the model and the transfer of the facility to the public. As a result of these evaluations and comparisons, It was concluded that the PPP model used in city hospitals was applied based on the example of England. In the study which was conducted with qualitative research method, document analysis data collection method was used.

İNGİLTERE KAMU-ÖZEL İŞBİRLİĞİ MODELİNİN TÜRKİYE’DEKİ ŞEHİR HASTANELERİ UYGULAMALARI ÜZERİNE ETKİLERİ: POLİTİKA TRANSFERİ BAĞLAMINDA BİR ANALİZ

Abstract

Sağlık sektöründe Kamu-Özel İşbirliği (KÖİ) modeli 1990'larda İngiltere'de uygulanmaya başlanmış ve bu ülkeden dünyaya yayılmıştır. KÖİ modelini uygulayan ülkelerin bazıları İngiltere örneğini doğrudan transfer etmiş, bazıları da kendi sağlık sistemlerine uyarlama yolunu tercih etmişlerdir. Bu bağlamda, sağlık sektöründe KÖİ “altyapı tabanlı model”, “ayrık klinik servisler modeli” ve “entegre KÖİ modeli” olmak üzere üç alt model şeklinde uygulanmaktadır. Bu çalışmanın amacı, İngiltere KÖİ örneği esas alınarak gerçekleştirilen şehir hastaneleri modelinin politika transferi bağlamında incelenmesidir. Bu amaçla, şehir hastanelerinde uygulanan KÖİ modeli çeşitli açılardan İngiltere KÖİ modeliyle karşılaştırılmaktadır. Karşılaştırma, özel sektör girişimcilerinin yapısı, kamu ve özel sektörün görev ve sorumluluk paylaşımı, özel sektöre ödenen bedeller, özel amaçlı şirketin kuruluşu ve işlevi, modelin uygulandığı sektörler, karşılaşılan riskler ve risklerin paylaşımları, modele yöneltilen eleştiriler ve tesisin kamuya devri başlıklarında ele alınmaktadır. Bu değerlendirmeler ve karşılaştırmalar sonucunda, şehir hastanelerinde kullanılan KÖİ modelinin İngiltere örneği esas alınarak uygulandığı sonucuna varılmıştır. Nitel araştırma yöntemi ile yapılan çalışmada, doküman incelemesi veri toplama yöntemi kullanılmıştır.
