The sensitivity of patch test in patients with psoriasis

Abstract

Objectives: Allergic diseases play an important role in the natural course of psoriasis. Atopic sensitization and contact dermatitis are common in patients with psoriasis.Since the symptoms are prolonged in patients who are resistant to therapy and exposure to itchy and external factors are common among these patients, the effects of contact allergens on triggering psoriasis are investigated. Contact allergens have an important role in activation and remission of psoriasis. We aimed to investigate contact sensitization rates in patients with psoriasis in the study. Material and Methods: Contact sensitization was investigated with the application of European standard series in twenty patients with psoriasis, twenty patients with contact dermatitis, and twenty healthy persons. Results: Among the whole study cases, positivity rate of patch test against one allergen at least was 25%. rate of patch test was 25% in patients with psoriasis, 35% in patients with contact dermatitis, and 15% in healthy persons. There were no signifi cant differences between the groups according to sensitization to one or more allergens (p>0.05). There were no signifi cant difference in clinical subgroup of psoriatic patients according to contact sensitization (p>0.05). The allergens in patients with psoriasis on patch test were as the followings: phenyldiamine, potassium dichromat, nickel, and cobalt. Conclusion: We think that the patch test has a major role in the diagnosis and elimination of allergens in patients with the chronic and resistant diseases and palmoplantar and fl exural psoriasis.

Psöriazis hastalarında yama testinin duyarlılığı

Abstract

Amaç: Psoriazisin doğal seyrinde allerjik hastalıklar önemli bir rol oynamaktadır. Psoriazis hastaları arasında atopik sensitizasyon ve kontakt dermatit sık görülmektedir. Tedaviye dirençli hastalarda sikâyetlerinin uzun süreli olması, kasıntılı ve dıs etkenlere devamlı maruz kalınması gibi nedenlerle kontakt allerjenlerin psoriazis üzerine tetikleyici etkisi arastırmalarda tartısılmaktadır. Kontakt allerjenlerin psoriazisin aktivasyonu ve devamında önemli rol oynamaktadır. Bu çalısmada psoriazis hastalarında yama testinin kontakt duyarlılığını saptanmayı amaçlanmıstır. Gereç ve Yöntemler: Yirmi psoriazis hastası ile yirmi kontakt dermatit hastası ve yirmi sağlıklı bireye ‘\'Avrupa standart serisi\'\' ile yama testi uygulandı. Bulgular: Tüm hastalarda, bir veya birden fazla allerjene karsı yama testi pozitifl iği oranı %25 olarak bulundu.Yama testi pozitifl iği oranı psoriazis hastalarında %25,kontakt dermatit hastalarında %35 ve sağlıklı kisilerde %15 olarak saptandı. Hem bir allerjen hem de birden fazla allerjene karsı duyarlılık açısından gruplar arasındaki farklılıklar önemsizdi (p>0.05). Psoriazis hastalarında klinik tipler arasındaki kontakt duyarlılıkları bakımından farklılıklar önemsizdi (p>0.05). psoriazis hastalarında yama testinde saptanan allerjenler fenilendiamin, potasyum dikromat, nikel ve kobaltdı. Sonuç: Uzun süreli hastalık öyküsü olan psoriazis hastaları, palmoplanter ve fl eksural psoriazis hastaları ve tedaviye dirençli psoriazis hastalarında allerjenlerin tespiti ve uzaklastırılmasında yama testinin önemli olduğunu düsünmekteyiz.
