NUTRİENT-DRUG İNTERACTİONS FROM A PUBLİC HEALTH PERSPECTİVE

Abstract

Nutrient-drug interactions refer to the interaction of foods, nutrients, or the nutritional status of an individual with any medication they are taking. Globally, the frequency of nutrient-drug interactions varies significantly, ranging from 6% to 70%. Drug components can affect a person's nutritional status, and similarly, substances found in food can influence the effectiveness of drugs. These interactions have direct implications for individual and public health. The objective of this study is to provide fundamental insights into nutrient-drug interactions, address related issues, and develop solutions from a public health perspective. In April and May 2023, scientific literature search on nutrient-drug interactions and their impact on public health was conducted. National and international publications were investigated. The keywords "food-drug interactions," "drug-nutrition status," and "public health" were utilized in both English and Turkish. Nutrient-drug interactions can occur through four main mechanisms, with some of these interactions posing life-threatening risks. Commonly used drugs in daily life, such as antihypertensives, warfarin, proton pump inhibitors, antibiotics, and nutritional supplements, play a significant role in nutrient-drug interactions. The causes of these interactions have been linked to various factors, including structural, social, economic, and commercial aspects that affect the individual and the social consequences affecting health. Certain periods and groups are at higher risk of experiencing nutrient-drug interactions, including the elderly, pregnant individuals, those with chronic diseases, and cancer patients. Individual risks and threats can have significant implications at the societal level. To address the problems arising from nutrient-drug interactions, interventions aligned with the primordial, primary, secondary, and tertiary levels of public health protection are necessary. In addition to the preventive strategies to be taken by public authorities at the community level, supporting approaches such as raising awareness among individuals, the cooperation of relevant stakeholders, and the development of evidence-based scientific research should be provided.

HALK SAĞLIĞI PERSPEKTİFİYLE BESİN-İLAÇ ETKİLEŞİMLERİ

Abstract

Besin-ilaç etkileşimi; besinler, besin ögeleri ya da kişinin beslenme durumu ile herhangi bir ilaç arasında meydana gelen etkileşimlerdir. Dünyada görülme sıklığı %6 ile %70 arasında değişmektedir. İlaçlarda bulunan çeşitli maddeler kişinin beslenme durumunu etkileyebildiği gibi, besinlerde bulunan maddeler de ilacın aktivitesini etkilemekte, bu durum birey ve toplum sağlığını yakından ilgilendirmektedir. Bu çalışmanın amacı besin-ilaç etkileşimi konularında temel bilgileri açıklamak ve halk sağlığı yaklaşımıyla sorunları saptamak, çözüm önerileri geliştirmektir. Bu makale için; besin-ilaç etkileşimi ve halk sağlığı konuları hakkında bilimsel literatür bilgilerine Nisan ve Mayıs 2023 döneminde ulaşılmıştır. Ulusal ve uluslararası yayınlar incelenmiştir. Bu amaçla Türkçe ve İngilizce olarak “besin-ilaç etkileşimi”, “beslenme durumu”, “ilaç kullanımı” anahtar sözcükleri kullanılmıştır. Besin-ilaç etkileşimi, dört farklı mekanizmada, onlarca besin-ilaç arasında görülebilmekte, bu etkileşimlerin bir kısmı hayati tehlike oluşturabilmektedir. Antihipertansifler, warfarin, proton pompa inhibitörleri, antibiyotikler gibi günlük yaşamda sık kullanılan ilaçlar ve besin takviyeleri, besin-ilaç etkileşiminin önemli bir kısmını oluşturmaktadır. Etkileşimlerin nedenleri ise yapısal, sosyal, ekonomik ve ticari yönden birçok faktörle ilişkilendirilmiş olup, sağlığı yine bu faktörler aracılığıyla etkileyen bireysel ve toplumsal sonuçlara yol açtığı gösterilmiştir. Besin-ilaç etkileşimi açısından yaşlılık, gebelik dönemi, kronik hastalıklar, kanser hastaları gibi riskli dönem ve gruplar bulunmaktadır. Bireysel düzeyde oluşan risk ve tehditler toplumsal düzeyde de önemli sorunlara yol açabilmektedir. Besin-ilaç etkileşimlerinin yarattığı sorunları önleyebilmek için halk sağlığının primordial, primer, sekonder ve tersiyer korunma düzeyleri ile uyumlu adımlara ihtiyaç vardır. Kamu otoritelerinin alacağı toplumsal düzeyde önlemlerin yanı sıra bireyler arasında farkındalığın artırılması, ilgili bileşenlerin iş birliği, kanıta dayalı bilimsel araştırmaların geliştirilmesi gibi özel yaklaşımların da desteklenmesi önerilmektedir.
