Partial necrosis of caecum mimicking acute appendicitis

Abstract

Objective: Partial caecum necrosis not related to trauma or tumor is a rarely encountered pathology in elderly population. It presents with appendicitis like abdominal pain in right lower quadrant. It is usually diagnosed peroperatively. Treated with partial resection or right hemicolectomy with early recognition and treatment it s morbidity and mortality is relatively low. Case report: Two patient 55 years old man end 67 years old woman after laparotomy with the diagnosis of acute appendicitis, upon detection of partial necrosis of the caecum. The male patient right hemicolectomy was performed. The other patient partial caecum resection and tube caecostomy was performed. Two patients were discharged without any complication. Conclusion: In the presence of acute appendicitis in elderly patients should be considered in partial necrosis of the caecum

AKUT APANDİSİTİ TAKLİT EDEN PARSİYEL ÇEKUM NEKROZU

Abstract

Amaç: Travma veya tümöre bağlı olmayan izole parsiyel çekum nekrozu yaşlılarda nadir görülen bir tablodur. Apandisit gibi sağ alt kadrana lökalize ağrı ile karekterizedir ve genellikle ameliyat esnasında tanı konur. Parsiyel çekum rezeksiyonu veya sağ hemikolektomi ile tedavi edilir. Erken teşhis ve tedavi edilirse morbidite ve mortalitesi düşüktür. Olgu sunumu: 55 yaşında erkek ve 67 yaşında bayan hastaya akut apandisit ön tanısıyla laparatomi yapıldı ve çekum nekrozu tespit edildi. Erkek hastaya sağ hemikolektomi diğer hastaya ise çekum rezeksiyonu ve tüp çekostomi yapıldı. İki hastada da herhangi bir komplikasyon olmadı. Sonuç: Akut apandisit kliniği ile gelen yaşlı hastalarda parsiyel çekum nekrozu da düşünülmelidir
