End-stage renal failure is an irreversible deterioration in the fluid-electrolyte balance and metabolic-endocrine functions of the kidney. Patients with end-stage renal failure are treated with three different renal replacement treatment methods: hemodialysis, peritoneal dialysis and kidney transplantation. Kidney transplantation, one of these treatments, significantly decreases the risk of mortality and morbidity compared to dialysis treatment, improves quality of life, and provides an independent living space for recipients. However, in kidney transplant patients, rejection, an undesirable condition, may develop in the days and weeksfollowing the transplant. Transition refers to the shift of roles from a known situation to another unknown situation. The most important role of nurses is to assist individuals experiencing transition and help them complete a healthy transition. Thisperiod requires the individual to adapt to a new role and situation. For these reasons, this article examines the case report and nursing care of a patient who previously received peritoneal dialysis treatment and was recommended hemodialysis treatment following rejection after kidney transplantation, in the light of Meleis's transition theory.
Son dönem böbrek yetmezliği; böbreğin, sıvı-elektrolit dengesi ve metabolik-endokrin fonksiyonlarında geri dönüşümü olmayan bir şekilde bozulması durumudur. Son dönem böbrek yetmezliğinin tedavisinde hemodiyaliz, periton diyalizi ve böbrek nakli gibi üç farklı renal replasman tedavi yöntemi kullanılmaktadır. Böbrek nakli, diyaliz tedavisine göre mortalite ve morbidite riskinde belirgin düşüş sağlamakta, yaşam kalitesini iyileştirmekte ve alıcılara bağımsız bir yaşam alanısağlamaktadır. Ancak böbrek nakli olan hastalarda nakil sonrası günler veya haftalar içerisinde istenmeyen bir tablo olan rejeksiyon gelişebilmektedir. Geçiş ise bilinen bir durumdan, bilinmeyen başka bir duruma rol geçişini ifade etmektedir. Hemşirelerin buradaki en önemli rolü, hastaların yeni bir duruma ve role sağlıklı bir geçişi tamamlamalarına yardımcıolmaktır. Bu olguda periton diyalizi tedavisinde iken böbrek nakli olan nakil sonrası rejeksiyon gelişen ve ardından hemodiyaliz tedavisi önerilen olgunun Meleis’in geçiş kuramına göre hemşirelik bakımı incelenmiştir.
