                                                                                                <abstract><p>Amaç: Koroner arter hastalığı, dünyada ölüm nedenleri arasında ilk sırada yer almaktadır. Günümüzde koroner arter bypass cerrahisi kabul edilebilir morbidite ve mortalite ile yapılmaktadır. Ancak yüksek riskli hastalarda kardiyopulmoner bypassın komplikasyonlarından sakınmak için, atan kalpte bypass işlemleri yaygın olarak yapılmaktadır. Bu çalışmada kliniğimizde atan kalpte bypass cerrahisi uygulanan hastaları sunmayı planladık.Gereçler ve Yöntem: Nisan 2012-Nisan 2023 tarihleri arasında kliniğimizde atan kalpte bypass uygulanan 72 hasta retrospektif olarak incelendi. Olgular yaş, cinsiyet, ek hastalıklar, sol ventrikül ejeksiyon fraksiyonu, bypass yapılan damar sayısı, postop inotrop ihtiyacı, kanama, yoğun bakımda ve hastanede kalış süreleri, revizyon ihtiyacı ve mortalite açısından değerlendirildi.Bulgular: Hastaların 26’sı(%36,11) kadın, 46’sı (%63,9) erkekti. Yaş ortalaması 63.36±8.5 yıl idi. 8 hastada kronik obstriktif akciğer hastalığı (KOAH) (%11,11), 33 hastada hiperlipidemi (HL) (%45,83), 25 hastada diyabetus mellitus (DM) (%34,72), 36 hastada hipertansiyon (HT) (%50) ve 26 hastada sigara kullanımı (%36,11) mevcuttu. 1 hastaya 5 damar bypass, 3 hastaya 4 damar bypass, 6 ahstaya 2 damar bypass, 27 hastaya 2 damar bypass ve 35 hastaya tek damar bypass uygulandı. 7 hasta acil olarak operasyona alındı. Bu hastalardan 2’sinde intraaortik balon pompası (İABP) kullanıldı. İABP kullanılan hastalardan biri sternum açık olarak yoğun bakıma alındı, postoperatif 4. gününde stabil seyretmesi üzerine sternum kapatıldı, postoperatif 6. gününde ekstübe edildi. İABP kullanılan diğer hasta postoperatif 1. günde exitus oldu. Sadece bu hastada mortalite görüldü.Sonuç: Yüksek riskli hasta grubunda atan kalpte koroner arter bypass uygulamaları düşük morbidite ve mortalite ile yapılmaktadır. Ancak uygulanabilirliği için iyi bir cerrahi deneyim ve uygun koroner anatomi gerekmektedir.</p></abstract>
