Fatigue and Assessment Methods in Neurological Disease

Abstract

Fatigue, which is frequently seen in neurological diseases, is a symptom that restricts the daily life activities of individuals and leads to a decrease in their quality of life. In the subjective evaluation of fatigue, many scales have been developed to investigate dimensions such as the severity of fatigue, its effects on daily life activities, and its social, cognitive, and psychological effects. Scales developed to measure fatigue specific to neurological diseases are inadequate in number. Most of the scales used to evaluate fatigue in neurological diseases have been developed to evaluate fatigue in different populations. Invasive methods and non-invasive physiological tests are used in the objective evaluation of fatigue. Blood test and muscle biopsy are invasive methods which used in the evaluation of fatigue. The need for good muscle activation in evaluating fatigue with non-invasive physiological tests such as electromyography is a factor limiting the use of these methods in neurological diseases. However, the fact that fatigue is a symptom that can exist independently of activity in neurological diseases and its subjective nature shows that objective assessment methods do not provide sufficient information about fatigue in neurological diseases. For these reasons, it is necessary to develop fatigue assessment tools specially developed for neurological diseases, which take into account the nature of the disease, factors affecting fatigue and pathology, and good psychometric properties. Studies on this subject will benefit clinicians and researchers working in the field of neurological rehabilitation.

Nörolojik Hastalıklarda Yorgunluk ve Değerlendirme Yöntemleri

Abstract

Nörolojik hastalıklarda sıklıkla görülen yorgunluk, bireylerin günlük yaşam aktivitelerinin kısıtlanmasına ve yaşam kalitelerinin azalmasına yol açan bir semptomdur. Yorgunluğun sübjektif değerlendirilmesinde yorgunluğun şiddeti, günlük yaşam aktiviteleri üzerine etkileri, sosyal, bilişsel ve psikolojik etkileri gibi boyutlarının araştırılması amacıyla birçok ölçek geliştirilmiştir. Nörolojik hastalıklara özgü yorgunluğu ölçmek için geliştirilmiş ölçekler sayıca yetersizdir. Nörolojik hastalıklarda yorgunluğu değerlendirmek amacıyla kullanılan ölçeklerin çoğu farklı popülasyonlarda yorgunluğu değerlendirmek için geliştirilmiştir. Yorgunluğun objektif olarak değerlendirilmesinde ise invaziv yöntemler ve invaziv olmayan fizyolojik testler kullanılmaktadır. Kan testi ve kas biyopsisi yorgunluğun değerlendirilmesinde kullanılan invaziv yöntemlerdendir. Elektromiyografi gibi invaziv olmayan fizyolojik testlerle yorgunluğun değerlendirilmesinde iyi derecede kas aktivasyonu gerekliliği, nörolojik hastalıklarda bu yöntemlerin kullanımını sınırlandıran bir faktördür. Bununla birlikte, nörolojik hastalıklarda yorgunluğun aktiviteden bağımsız olarak da var olabilen bir semptom olması ve sübjektif doğası objektif değerlendirme yöntemlerinin nörolojik hastalıklarda yorgunluk ile ilgili yeterli bilgi vermediğini göstermektedir. Bu nedenlerle, nörolojik hastalıklara özel geliştirilmiş, hastalığın doğasını, yorgunluğu etkileyen faktörleri ve patolojisini göz önünde bulunduran ve psikometrik özellikleri iyi olan yorgunluk değerlendirme araçlarının geliştirilmesi gerekmektedir. Bu konuda yapılacak olan çalışmalar, nörolojik rehabilitasyon alanında çalışan klinisyen ve araştırmacılara fayda sağlayacaktır.
