Oral health–related quality of life improvement following full-mouth implant rehabilitation in stage III/IV periodontitis patients

Abstract

Aims: Periodontitis is a chronic inflammatory condition that significantly impairs oral function and quality of life. In advanced stages (stage III/IV), full-mouth extraction followed by implant-supported prosthetic rehabilitation is often necessary, yet limited data are available regarding the effect of this transition on oral health–related quality of life (OHRQoL). To assess the changes in OHRQoL among patients with stage III/IV periodontitis who underwent complete tooth extraction and full-arch implant-supported prosthetic rehabilitation, and to evaluate the impact of clinical and demographic variables on patient-reported outcomes. Methods: In this longitudinal study, 54 patients diagnosed with stage III/IV periodontitis were enrolled. Clinical parameters and OHRQoL were assessed using the Turkish version of the Oral Health Impact Profile-14 (OHIP-14). Measurements were recorded at two time points: at baseline (T0), when patients retained their natural dentition and clinical assessments were obtained from periodontal tissues, and at follow-up (T1), two years after completion of full-mouth implant-supported rehabilitation, when clinical assessments were performed on peri-implant tissues. Statistical analysis involved paired t-tests and multiple linear regression to evaluate changes and influencing factors. Results: A significant improvement in OHIP-14 scores was observed, decreasing from 44.96±11.63 at baseline to 16.81±8.61 after treatment (p<0.001). Clinical parameters (probing depth, attachment level, and bleeding on probing) also showed significant changes over time, with baseline measurements obtained from periodontal tissues (T0) and follow-up measurements reflecting peri-implant tissues (T1). Among evaluated factors, only prosthesis type significantly influenced follow-up OHIP-14 scores, with fixed prostheses associated with better outcomes (β=0.286, p=0.044). Conclusion: Full-arch implant-supported rehabilitation significantly enhances OHRQoL in patients with advanced periodontitis. Prosthetic design, especially the choice of fixed over removable prostheses, is crucial in achieving the best patient-reported outcomes.

Evre III/IV reriodontitis hastalarında tam ağız implant rehabilitasyonunun ağız sağlığı ile ilişkili yaşam kalitesine etkisi

Abstract

Giriş: Periodontitis, oral fonksiyonu ve yaşam kalitesini önemli ölçüde olumsuz etkileyen kronik inflamatuar bir durumdur. İleri evrelerde (Evre III-IV) tam ağız diş çekimi ve ardından implant destekli protetik rehabilitasyon sıklıkla gerekli olmaktadır; ancak bu geçişin ağız sağlığı ile ilişkili yaşam kalitesi (OHRQoL) üzerindeki etkisine yönelik veriler sınırlıdır. Amaç: Evre III–IV periodontitis hastalarında tüm dişlerin çekimi ve tam ark implant destekli protetik rehabilitasyon sonrasında OHRQoL’deki değişiklikleri değerlendirmek ve klinik ile demografik değişkenlerin hasta bildirimli sonuçlar üzerindeki etkisini incelemek amaçlanmıştır. Gereç ve Yöntem: Evre III–IV periodontitis tanısı konulan 54 hasta çalışmaya dahil edildi. Ağız Sağlığı Etki Profili-14 (OHIP-14) ölçeğinin Türkçe versiyonu tedavi öncesinde ve protetik rehabilitasyondan iki yıl sonra uygulandı. Periodontal parametreler ve sosyodemografik veriler kaydedildi. Değişiklikleri ve etkileyen faktörleri değerlendirmek için t-testi ve çoklu lineer regresyon analizleri kullanıldı. Bulgular: OHIP-14 skorlarında anlamlı bir iyileşme gözlendi; başlangıçta 44.96±11.63 olan skorlar tedavi sonrasında 16.81±8.61’e geriledi (p<0.001). Periodontal parametrelerde (sondlama derinliği, klinik ataşman kaybı ve sondlamada kanama) da belirgin iyileşme kaydedildi. Değerlendirilen faktörler arasında yalnızca protez türü tedaviler sonrası OHIP-14 skorlarını anlamlı olarak etkiledi; sabit protezler daha iyi sonuçlarla ilişkili bulundu (β=0.286, p=0.044). Sonuç: Tam ark implant destekli rehabilitasyon, ileri periodontitisli hastalarda OHRQoL’yi anlamlı düzeyde iyileştirmektedir. Protetik tasarım, özellikle sabit protez tercih edilmesi, en iyi hasta bildirimli sonuçların elde edilmesinde kritik bir rol oynamaktadır.
