Akut koroner sendromlarda (miyokard infarktüsü, kararsızangina) intrakoroner trombüs görülme oranı yüksektir. Miyokard infaktüsü ilegelen hastalarda intrakoroner trombüs mevcudiyeti güncel müdahale tekniklerinerağmen işlem başarısını düşürmekte ve çeşitli komplikasyonlara nedenolmaktadır. Bizim vakamızda 49 yaşında erkek hastada gelişen komplikasyonlar veyönetimi sunulmuştur. ST elevasyonlu miyokard infarktüsü ile gelen hastanın sonön inen koroner arterde trombüsün aspirasyonu sonrası tüm sol sistemde yaygıntrombüs gözlemlendi. İntrakoroner heparin ve tirofiban sonrasında trombüsgeriledi ve direkt stent implantasyonu ile kompikasyon başarılı bir şekildeyönetildi. Son kılavuzlarda da gösterildiği üzere trombüs aspirasyonu, yoğunkoroner arter trombüsü olması durumunda mortaliteyi istatistiksel olarakanlamlı olmasa da azalttığı gösterilmiş ancak koroner arter trombüs aspirasyonurutin olarak önerilmemektedir.

The incidence of intracoronary thrombosis is high inacute coronary syndromes. Despite the current intervention techniques, thepresence of intracoronary thrombus in patients with myocardial infarction,decreases the success of the procedure and causes various complications. Wepresent the complications and management in a 49-year-old male patient. Acommon thrombus was observed in the left whole system after aspiration of thethrombus in the left anterior descending coronary artery of the patient with STelevation myocardial infarction.Thrombosis regressed after intracoronary heparin andtirofiban, and the complication was successfully managed with direct stentimplantation. Thrombus aspiration has been shown to reduce mortality in thecase of intense coronary artery thrombosis, although not statisticallysignificant, as shown in recent guidelines, but coronary artery thrombusaspiration is not routinely recommended.
