MELKERSSON-ROSENTHAL SYNDROME WHICH APPEARS WITH SIMULTANEOUS BILATERAL FACIAL PARALYSIS: A RARE CASE

Abstract

Melkersson-Rosenthalsyndromeis classically described as a triad of recurrent orofacial edema, recurrent facial palsy and fissured tongue. In this syndrome, recurrence of facial paralysis usually may occur on the same side or opposite side at different times. But the presented 56 year-old male patient, unlike classical signs, showed a bilateralfacialparalysissimultaneously.

SİMULTANE BİLATERAL FASİYAL PARALİZİ İLE SEYREDEN MELKERSSONROSENTHAL SENDROMU: NADİR BİR VAKA

Abstract

Melkersson–Rosenthal sendromu rekürren fasiyal paralizi, rekürren orofasiyal ödem ve fissürlü dil bulguları triadı ile seyretmektedir. Bu sendromda fasiyal paralizi rekürrens göstermekle beraber karşı tarafta ya da genellikle aynı tarafta farklı zamanlarda meydana gelebilmektedir. Ancak sunulan 56 yaşındaki erkek hastada klasik bulgulardan farklı olarak, fasiyal paralizinin bilateral ve eşzamanlı olarak meydana geldiği gösterilmiştir.Melkersson-Rosenthal Sendromu (MRS) rekürren fasiyal paralizi, plikalı veya fissürlü dil ve üst dudakta ağırlıklı olmak üzere orofasiyal ödem ile karakterize nadir granülomatöz bir hastalıktır(1,2). Etiyolojisi tam bilinmemektedir ancak histopatolojik olarak multinükleer dev hücreler, artmış inflamatuar hücreler ile birlikte dilate lenfatik kanallar gözlenmektedir(3). Melkersson, 1928’de fasiyal ödem ve fasiyal paralizili bir hastada bu sendromu tanımlamıştır. Rosenthal, sonraki yıllarda bu sendromun lingual fissür ayağını tarif etmiştir. Bazı çalışmalarda bu sendromun ailesel ve otozomal dominant kalıtıldığı bildirilmiştir(4).
