Göçmen Kadınlarda Üreme Sağlığı

Abstract

Birleşmiş Milletler, Suriye krizini bir halk sağlığı felaketi ve 21. yüzyılın en büyük insani krizlerinden biri olarak nitelendirmiştir. Zorla yerinden edilme can kaybına, yoksulluğa, artan fiziksel ve ruhsal hastalıklara neden olup, üreme sağlığı, doğum öncesi ve doğum sonrası bakım ve çocuk sağlığı gibi sürdürülebilir sağlık hizmetlerinde aksamalara yol açmaktadır. 2011 tarihinden bugüne Suriye’deki savaştan kaçarak ülkemize sığınan kişilerin sayısı 2021 ağustos ayı itibari ile 3.701.584 olup, yaklaşık yarısını kadınlar oluşturmaktadır. Göç sebebinden bağımsız olarak göçten en fazla etkilenen gruplar kadınlar ve çocuklardır. Etnik yapı, kültür, cinsiyet ayrımcılığı, geleneksel yaşam kalıpları ve dini inanış kadınların göçe bağlı olumsuz etkileri daha fazla yaşamasına sebep olmakta ve kadınların sağlık hizmetlerine ulaşımında engel oluşturabilmektedir. Sağlık hizmetlerine erişimdeki zorluklar adölesan gebelikler, karşılanmayan aile planlaması gereksinimleri, maternal hastalıklar, anne ölümleri, cinsel yolla bulaşan hastalıklar, sağlıksız koşullardaki doğumlar ve düşükler gibi üreme sağlığı problemlerine neden olmaktadır. Mülteci kadınlarda üreme sağlığı gerekliliklerinin yeterice karşılanamaması anne, bebek ve toplum üçlüsü için sağlık açısından kötü sonuçlara neden olabilmektedir. Mülteci kadınların üreme sağlığı sorunları dikkatle ele alınması gereken önemli bir konudur. Üreme sağlığı hizmetleri ve tüm koruyucu sağlık hizmetleri kolay ve devamlı erişebilir olmalıdır. Mülteci kadınlarda, 15-49 yaş arası izlemlerin, doğum öncesi ve sonrası bakımların düzenli yapılabilmesi, adölesan dönem evliliklerin ve gebeliklerin önlenmesi için temel ihtiyaçların karşılanmasına yönelik olarak sağlığı geliştirme çalışmalarının planlanması yapılmalıdır. Tecrübeli sağlık çalışanlarının bu alanlarda aktif rol oynaması ve bu hizmetlerin eğitimlerle desteklenmesi mevcut sorunların giderilmesinde olumlu katkı sağlayacaktır. Sağlık çalışanlarının göçmen kadınların üreme sağlığı sorunlarına yönelik olarak farkındalıklarının artırılması, mevcut sorunların tespit edilmesi ve çözülmesini kolaylaştıracaktır.

Reproductive Health in Migrant Women

Abstract

The United Nations has characterized the Syrian crisis as a public health disaster and one of the greatest humanitarian crises of the 21st century. Forced displacement causes loss of life, poverty, increased physical and mental illness, and disrupts sustainable health services such as reproductive health, prenatal and postnatal care, and child health. The number of people who took refuge in our country by escaping from the war in Syria since 2011 is 3,701,584 as of August 2021, and approximately half of them are women. Regardless of the reason for migration, the groups most affected by migration are women and children. Ethnicity, culture, gender discrimination, traditional life patterns and religious beliefs cause women to experience more negative effects of migration and can create barriers for women to access health services. Difficulties in accessing health services cause reproductive health problems such as adolescent pregnancies, unmet need for family planning, maternal diseases, maternal deaths, sexually transmitted diseases, births in unsanitary conditions and abortions. Failure to adequately meet the reproductive health requirements of refugee women can lead to adverse health outcomes for the mother, baby and the trio of society. Reproductive health problems of refugee women is an important issue that needs to be handled carefully. Reproductive health services and all preventive health services should be easily and continuously accessible. Health promotion activities should be planned to meet the basic needs of refugee women between the ages of 15-49, regular prenatal and postnatal care, and prevention of adolescence marriages and pregnancies. Experienced healthcare professionals playing an active role in these areas and supporting these services with training will contribute positively to the elimination of existing problems. Increasing the awareness of health workers about reproductive health problems of migrant women will facilitate the identification and resolution of existing problems.
