ACİL SERVİSE DERMATOLOJİK YAKINMALAR İLE BAŞVURAN HASTALARIN ANALİZİ

Abstract

Amaç:  Çalışmamız Acil Servise dermatolojik
yakınmalarla başvuran hastaların demografik ve klinik özellikleri,
konsültasyon, taburculuk ve hastaneye yatış oranları ile hayati tehlike arz
edebilecek klinik tabloları hakkında bilgi vermek amaçlanmıştır.



Yöntem:
Erciyes Üniversitesi Tıp Fakültesi Acil Servisi’ne 01.08.2013 ile 01.02.2014
tarihleri arasında dermatolojik şikayetlerle başvuran hastalardan 16 yaş ve
üzeri olanlar dahil edilerek prospektif olarak yapılmıştır. Değerlendirmede
yaş, cinsiyet, özgeçmiş, başvuru yakınması ve ortaya çıkış zamanı, hastaneye
başvurana kadar geçen süre, lezyonun tipi, tanısı, konsültasyonlar, hastaneye
yatış ve taburculuk durumunu içeren parametreler yer almıştır.



Bulgular:
Çalışmaya toplam 400 hasta dahil edilmiştir. Acil Servise başvuran hastaların %
0,70’ini dermatolojik yakınmalar ile gelenler oluşturuyordu. Hastaların 221’i
(%55) kadın ve yaş ortalaması 38.6 yıl olup, 179’u (%45) erkek ve yaş
ortalaması 39.1’di. Ciltte kızarıklık % 29.8 oranı ile en sık başvuru nedeni ve
ürtiker en çok tesbit edilen tanıdır (%54.3). 
Hastaların %48.3’ünün yakınmalar başladıktan en az 24 saat sonra Acil
Servise başvurmuştur. %31’i konsülte edilmiş olup en sık nedenler; sellülit ve
anjioödemdir. ‘Gerçek dermatolojik acil’ olarak nitelendirilen nekrotizan
fasiit, toksik epidermal nekrolizis, eritema multiforme birer hastada
görülmüştür (%0.75). Hastaların % 19’unun hastaneye yatırılmıştır ve en sık
tanı sellülittir (%30.8). Lezyon vücut yüzey alanının tek başına hastaneye
yatış oranını etkilemediği görülmüştür.



Sonuç:
Dermatolojik yakınmalar ile başvuran hasta sayısının, tüm Acil Servis
başvuruları içindeki oranı az olsa da, özellikle yaşamı tehdit edebilecek
sistemik organ etkilenmesi olan hastaların hastaneye yatırılarak tedavi
edilmesi daha uygundur. Taburcu olan hastalara, yakınmalarına yönelik etyolojik
nedenlerin araştırılması ve tedavinin etkinliğinin izlenmesi açısından
Dermatoloji polikliniğine başvurması önerilmelidir.

EVALUATION OF PATIENTS WHO REFERRED TO EMERGENCY DEPARTMENT WITH DERMATOLOGIC COMPLAINTS

Abstract

Aim:
In this study, we aimed to identify requirements of consultation, discharge and
hospitalization rates, life threatening clinical conditions; clinical and
demographic characteristics of patients who referred to Emergency Department
with dermatologic complaints.

Methods:
In this prospective designed study 16 years old or elder patients who referred
to Emergency Medicine Department. Many parameters such as age, gender, medical
history, complaints on referral and symptoms related to them, duration of complaints,
time to referral, presence of any dermatologic lesion, predisposing factors,
consultations with dermatology and other departments through the evaluation
process, eventual diagnosis, discharge and hospitalization were evaluated.

Results:
400 patients were enrolled to the sudy. Proportion of patients with
dermatologic complaints to all emergency referrals was % 0.7.  221 of patients were female with mean age
38.6  years and 179 of patients  were male with mean age 39.1 years. 107 of
patients were employees and 293 of patients were not working at all. Rush at
any site of body was the most common complaint and with % 54.3 rate urticaria
was defined as the most frequent eventual diagnosis. Referral within the 6
hours was limited with % 20.3 of patients, and in this subgroup majority of
diagnoses were composed by urticaria, angioedema and drug eruption.







Conclusion:
Patients referred to Emergency Department with dermatologic complaints were
estimated as a small group among all emergency patients. Majority of patients
were discharged with prescriptions and suggestions. Emergency physicians should
be alerted on early hospitalization and consultation requirements while
managing patients who are presented with sore, ulcerated or bullous lesions
with cautious.
