Bağ Kesen Total Diz Protezinde Post Geometrisinin Konveks ve Konkav Durumunun Aşınmaya Etkisinin SEA ile Araştırılması

Abstract

Diz
osteoartritinde oluşan ağrı ve eklem hareket kısıtlılığını gidermede metal ve
polietilen malzemeden imal edilen diz eklem protezleri kullanılmaktadır. Bu
protezlerde hastanın yaşı, aktivite düzeyi, kilosu ve ek hastalıkları gibi
nedenlere bağlı olarak aşınma, gevşeme, dengesizlik ve enfeksiyon gibi
sorunlarla karşılaşılmaktadır. Bu sorunlara bağlı olarak da protezlerin
kullanım ömürleri değişmektedir. Protezlerde görülen bu sorunların çözümü için
protez tasarımları üzerine yoğun çalışmalar devam etmektedir. Bunlardan biri de
arka çapraz bağın görevini yapması için tasarlanan bağkesen total diz
protezidir.



     Bağkesen protezlerdeki post-cam değişik
geometrik şekillere göre dizayn edilmesine rağmen ideal tasarımın yapılması
üzerine çalışmalar devam etmektedir. Bu çalışmada post-cam’deki düz-düz,
konveks-konkav ve konkav-konveks olmak üzere üç farklı geometrik temas alanının
von misses gerilmesine ve kontak basıncına etkileri sonlu elemanlar analizi ile
araştırılarak aşınma üzerindeki etkileri incelendi. Çalışma sonucunda temas
yüzeyi alanının artmasının von misses gerilmesi ve kontak basıncını azalttığı
ve buna bağlı olarak da aşınma miktarının azaldığı tespit edildi. Analiz
sonuçlarına göre de aşınmanın konkav-konveks temasa dayalı tasarımlarda en
düşük olduğu ölçüldü.
